Sepsis documentation and organ dysfunction criteria

Reimbursement requires appropriate coding and documentation demonstrating infection and organ dysfunction; otherwise claims may be denied.

ALL of the following

ONE of
- A reasonable suspicion of infection is described and documented as suspected at the time of discharge AND compatible clinical illness is documented.
- Infection is documented in the medical record to be clinically significant by one or more laboratory methods (not considered a contaminant), such as cultivation of bacteria or fungi on growth medium; detection of specific immune response (antibody); detection of antigens or nucleic acid sequences (e.g., immunohistochemistry, in situ hybridization, PCR); identification of the agent biochemically, antigenically, or genetically; isolation of viruses in cell culture; or visualization of an organism in infected tissue.

ALL of the following

ONE of
- Septic shock defined as requirement for vasopressors to maintain mean arterial pressure ≥65 mm Hg AND serum lactate level >2 mmol/L (greater than 18 mg/dL) in the absence of hypovolemia.
- An increase in the Sequential (Sepsis-related) Organ Failure Assessment (SOFA) score of 2 points or more, as reflected in the SOFA scoring tables and coding guidance.

ALL of the following

Sepsis should not be coded when the condition is being ruled out or is more appropriately explained by another diagnosis; providers must adhere to current coding and documentation guidelines and ensure claims accurately describe services rendered.
Exceptions may apply where a provider contract dictates otherwise; inpatient facility claims reimbursed under a methodology other than DRGs are addressed separately.
Violations of this policy may result in increased auditing, monitoring, contractual penalties, or termination; disciplinary actions determined by the Plan.

Claims for adults age 19 and older billed with a sepsis-related DRG may be denied unless documentation supports both suspected or proven infection AND organ dysfunction consistent with sepsis. Providers must ensure clinical documentation at the time of discharge explicitly states a reasonable suspicion of infection or documents a clinically significant infectious organism (not a contaminant), plus evidence of organ dysfunction.

Documentation must include ONE of the following: Suspicion of infection described and documented as suspected at the time of discharge, with a compatible clinical illness; OR infection documented as clinically significant by laboratory methods (culture or other definitive testing) and not considered a contaminant.
Must also document organ dysfunction consistent with sepsis (e.g., SOFA score changes, hypotension requiring vasopressors, acute kidney injury, acute respiratory failure, altered mental status, coagulopathy).
Failure to meet these documentation requirements may result in denial of the sepsis-related DRG claim for adults 19+.
Exceptions may apply where a provider contract dictates otherwise; inpatient facility claims reimbursed under a methodology other than DRGs follow contract terms.
