MEDICAL POLICY - 5.01.591 Immune Checkpoint Inhibitors
Policy governs medical necessity review for intravenous and injectable immune checkpoint inhibitor drugs, including site-of-service (SOS) criteria for age ≥13 and drug-specific indication coverage for multiple PD-1/PD-L1 and CTLA-4 agents. SOS criteria do not apply to Alaska fully-insured members. This extract is Part 1 of 4 and includes SOS rules and many drug-specific covered indications present in this part.
Added indication and coverage criteria for Loqtorzi (toripalimab-tpzi).
