Initiation of Powered NPWT (Medical Necessity)

An initial therapeutic trial of not less than 2 weeks using a powered negative pressure wound therapy (NPWT) system, as part of a comprehensive wound care program that includes controlling factors (e.g., diabetes, nutrition, relief of pressure), may be considered medically necessary in adults for ANY of the following indications:

ANY of the following

Chronic stage III/IV pressure ulcers
- Chronic (greater than 90 days) stage III or IV pressure ulcer that has failed to heal despite optimal wound care
- High-volume drainage that interferes with healing and/or when standard dressings cannot be maintained due to anatomic factors
  High-volume drainage is defined as exudate exceeding 150 - 200 mL/day that overwhelms NPWT dressings, breaks seal integrity, and hinders healing.
Nonhealing wounds in individuals with underlying conditions

Continuation of Powered NPWT (Medical Necessity & Not Medically Necessary)

Single-Use NPWT Systems — Investigational / Not Covered

General Not Medically Necessary: Therapeutic trials of powered NPWT systems for the treatment of other acute or chronic wounds that do not meet the specified initiation criteria are considered not medically necessary.

Background: NPWT uses negative pressure (suction) to aspirate and remove exudate, debris, and infectious material from the wound bed to promote granulation tissue formation and wound healing. The FDA has issued safety alerts (2009 and updated 2011) regarding serious complications, including bleeding and retained dressing pieces, and recommends careful patient selection and frequent monitoring. NPWT systems are FDA-cleared for use in adults but no devices are cleared for infants, children, or adolescents (no NPWT device cleared for those under 18). Include wound staging and duration, evidence of high-volume drainage when applicable, inability to maintain standard dressings due to anatomy, documentation of underlying comorbidities that impair healing, and for malnutrition document that treatment is being addressed simultaneously.
