Coverage Criteria for Prostate Biopsy Pathology

Covered indications

Covered when ALL of the following are met

Initial diagnosis biopsy: Pathological examination of tissue obtained from a prostate biopsy involving 12 core extended sampling is reimbursable for initial diagnosis following abnormal PSA, palpable nodule on digital rectal examination, or suspicious radiologic findings.

Follow-up (repeat) biopsy: Pathological examination of tissue from a follow-up prostate biopsy (excluding prostate saturation biopsy) is reimbursable when clinical suspicion persists after an initial negative biopsy.

Pathological examination of tissue obtained from a prostate saturation biopsy is specifically excluded from reimbursement. The policy states that saturation biopsy — extensive sampling obtaining up to 24 core samples — is not reimbursable for diagnosis, staging, or management of prostate cancer due to limited evidence of benefit and higher complication rates.

Per ESMO guidance referenced in the policy, when multiparametric MRI is negative (defined as PI-RADS ≤2) and clinical suspicion for prostate cancer is low, a biopsy can be omitted.

Saturation biopsy specimens are considered not medically necessary for diagnosis, staging, or management of prostate cancer and therefore are not reimbursable under this policy. The document notes saturation techniques may increase complications and are not appropriate for initial diagnostic use.
