Coverage Criteria for Zuranolone (Zurzuvae)

Zurzuvae (zuranolone) — Medical necessity

Covered when ALL of the following are met:

Zurzuvae (zuranolone) coverage criteria: 1) Individual is aged 18 years or older; 2) Medical record documents DSM-5 diagnostic criteria for Major Depressive Disorder with peripartum onset; 3) Current episode of depression is moderate to severe as demonstrated by documentation of individual's symptoms and their severity or by one or more standardized depression rating scales; 4) Individual is 12 months or less postpartum; 5) The treatment course is limited to 14 days.

Re-authorization of zuranolone beyond 14 days in a single treatment course per pregnancy is considered not medically necessary.

For individuals on strong CYP3A4 inhibitors, with severe hepatic impairment (Child-Pugh C), or severe renal impairment (eGFR <60 mL/min/1.73 m2), the recommended dose is 30 mg orally once daily in the evening for 14 days.
