Medical Necessity — Admission, Intervention, and Continued Stay

Admission Symptom Criteria (Medical Necessity)

All of the following must be present:

- Substance Use Disorder, moderate or severe, other than for tobacco/nicotine, not in remission
- The substance(s) in use have withdrawal syndromes that can be moderate, severe, potentially life‑threatening, can potentially cause serious physical harm, or can cause physical withdrawal symptoms that are so physically intolerable that withdrawal is highly unlikely to be completed without 24/7 containment (excludes simple or less severe physical or mental discomfort).
- Continuous use of the substance(s) such that discontinuation without active medical monitoring and management will produce a withdrawal syndrome at a level described above.
- At least one of the following must be present
  - Current signs or symptoms of moderate, severe, or potentially life‑threatening withdrawal
  - Current signs or symptoms indicate that moderate, severe, or potentially life‑threatening withdrawal is imminent without active medical management
  - Pattern and amount of recent/frequent substance use such that moderate, severe, or potentially life‑threatening withdrawal is highly likely with discontinuation without active medical management
  - Previous moderate, severe, or life‑threatening withdrawal from the same or similar substance(s) with a similar use pattern
  - Current signs or symptoms of physically severely intolerable withdrawal
  - Current signs or symptoms indicate that physically severely intolerable withdrawal is imminent without active medical management

Admission Location and Not Medically Necessary Locations

Residential and sub-acute detoxification may be considered medically necessary when provided in the following settings and not medically necessary in others:

ANY of the following

Medically necessary locations: stand-alone substance use disorder or dual disorders residential treatment facility; sub-acute treatment facility; psychiatric residential treatment facility; substance use disorder or dual disorders residential treatment unit in a general hospital; stand-alone detoxification facility (unless licensed as a hospital, in which case care is inpatient detoxification).
Not medically necessary locations: halfway house or other sober living residence; wilderness/adventure/camping program or expedition; boarding school or therapeutic boarding school; boot camp; military school or academy; group home; therapeutic home; supported or alternative housing.

Admission Intervention Criteria (required on admission / first day)

ALL of the following must be present for admission (intervention criteria):

ALL of the following

A physician, nurse practitioner, or physician assistant must conduct an admission evaluation, including medical history, documentation of withdrawal signs and symptoms, and physical examination, within one day after admission; must subsequently evaluate the patient at least once every day (in-person or by telehealth) assessing medical status, withdrawal signs and symptoms, medication regimen, and other continued treatment needs.
Substance use evaluation must be completed during the first day of admission.
Medication to manage or prevent withdrawal symptoms must be started during the first day of admission.
Active medical management must continue throughout the stay (except as allowed when medications are discontinued per criteria).
Active medical management is defined separately (see Definitions).

Definition of Active Medical Management (operational)

Active medical management means all of the following:

ALL of the following

Daily use of medication(s) to treat or prevent withdrawal symptoms.
Evaluation of the effects of such medication(s) at least once daily, with dose adjustments or medication changes when indicated.
Physical examination components as appropriate for the withdrawal or potential withdrawal syndrome at least once daily and more often when indicated.
Vital signs when indicated.
Laboratory testing as appropriate for the withdrawal or potential withdrawal syndrome when indicated.

Continued Stay Criteria (medical necessity for ongoing stay)

At least one of the following must be present to justify continued stay:

ANY of the following

Continued signs or symptoms of moderate, severe, or potentially life‑threatening withdrawal.
Continued signs or symptoms indicating that moderate, severe, or potentially life‑threatening withdrawal is likely without ongoing active medical management.
Continued signs or symptoms of physically severely intolerable withdrawal.
Continued signs or symptoms indicating that physically severely intolerable withdrawal is likely without ongoing active medical management.
Mild withdrawal signs and symptoms from alcohol or benzodiazepine withdrawal with active medical management, and less than 96 hours (4 days) since last reported use of alcohol or a benzodiazepine.

Continued Stay Not Medically Necessary (exclusions/contraindications)

Continued residential and sub-acute detoxification are not medically necessary in these circumstances:

ANY of the following

When patient is being observed for possible withdrawal signs or symptoms without active medical management ('social detoxification').
When continued stay is due to post-acute withdrawal syndrome (PAWS) symptoms (PAWS manageable outpatient up to 20 months).
When continued stay is because discharge planning has not been started or completed.
When continued stay is because the patient is waiting for a residence or treatment placement.
When continued stay is due to uncertainty about or lack of an adequate family, living environment, peer, community, or other support system.

Admission/Intervention Not Medically Necessary (exclusions)

Residential and sub-acute detoxification are not medically necessary for these situations:

ANY of the following

Admission due to a programmatic requirement or standard procedure that all patients are initially admitted for detoxification prior to residential treatment.
Observation for possible withdrawal signs or symptoms without active medical management ('social detoxification').
Ultra-rapid detoxification (anesthesia-assisted detoxification / anesthesia under heavy sedation).
Programs consisting of sauna detoxification or time in a sweat lodge.
Programs consisting of use of hot baths, hot tubs, or Jacuzzis without active medical management.

It excludes settings and methods that do not provide medical management (for example, halfway houses, wilderness/adventure programs, sauna or sweat-lodge programs, herbal or supplement-only programs, ultra-rapid/anesthesia-assisted detox, programs relying only on baths/exercise/hydration/nutrition) and excludes observation without active medical management ('social detoxification'). The policy emphasizes that detoxification in these settings must include active medical management (daily medications, daily medication effect evaluation with dose adjustment as needed, daily physical exam components, vital signs and laboratory testing when indicated) to be considered medically necessary.

Pattern and amount of recent/frequent substance use such that physically severely intolerable withdrawal is highly likely with discontinuation without active medical management

Previous physically severely intolerable withdrawal from the same or similar substance(s) with a similar use pattern

A court order for any designated length of stay does not make continued residential and sub-acute detoxification medically necessary; symptom and intervention criteria must be met.

Programs consisting of oral or parenteral vitamins and/or mineral supplementation and/or amino acid supplementation and/or other supplements without active medical management.

Isolated incident of acute intoxication (may warrant 'drying-out' in an observation bed rather than residential detoxification).

A court order alone does not make admission medically necessary; symptom and intervention criteria must be met.
