Medical necessity criteria for psychiatric and medical evaluations

Covered when ALL of the following are met (policy supplements InterQual criteria):

Timing requirement: Psychiatric evaluation performed within 24 hours (one day) of admission for inpatient settings; within one business day of admission for residential settings; and thereafter at least once every 7 days for residential or at the frequency specified by InterQual for inpatient continued care.timing per setting

Residential/sub-acute detoxification programs and Wilderness Therapy/Outdoor Behavioral Healthcare residential wilderness programs are excluded from this policy because separate policies specify the clinician types required for evaluations (see policies 3.01.515 and 3.01.522), and therefore these settings are not covered by the clinician-type rules in this document.

Evaluations performed by clinicians who hold only registration or certification but are not licensed clinical practitioners do not satisfy the policy requirement for psychiatric or psychiatric medication evaluations; registered/certified clinicians are explicitly distinguished from licensed clinical practitioners and cannot be used to meet the required documented evaluations.
