Coverage Criteria — USPSTF Preventive Services
USPSTF A/B preventive services
Covered when USPSTF recommendation grade is A or B.
Premera covers USPSTF A and B services in full in accordance with federal healthcare reform or at a reduced out-of-pocket cost for members enrolled in grandfathered plans. Verify member benefits via Premera's website or Customer Service.
Premera follows USPSTF grading when determining coverage. Services with USPSTF Grade A or Grade B recommendations are covered in accordance with the USPSTF guidance—these services are offered or provided because the USPSTF finds substantial (Grade A) or moderate (Grade B) net benefit. By contrast, the USPSTF assigns Grade D when it recommends against a service because there is moderate or high certainty that the service has no net benefit or that harms outweigh benefits; such services are not included among the USPSTF A/B preventive services and are discouraged. The USPSTF Grade I designation indicates evidence is insufficient to assess balance of benefits and harms; these services require clinical judgment and discussion with the patient because the net benefit is uncertain.
