Defines medical necessity criteria and prior authorization expectations for the appropriate site of service (ASC, on-/off-campus outpatient hospital, inpatient) for certain elective surgical procedures and which procedures this guideline applies to.
Site of Service for Ambulatory Service Center (ASC) for Select Surgical Procedures will be considered medically necessary when criteria are met.
