Preferred site: Ambulatory Surgical Center is the preferred medically necessary site for certain elective surgical procedures when accessible and appropriate.No qualifying ASC within 30 miles

Access and procedural reasons: No geographically accessible ASC within 30 miles that can provide the necessary care due to: lack of necessary equipment at any ASC, lack of ASC where the individual's physician has privileges, or ASC-specific guideline prohibiting use for the individual's health condition or weight.30 miles

Procedure categories: Breast surgery; Cardiac surgery (including implantable loop recorder procedures as added in history); Ear, Nose, Throat (ENT) surgery including rhinoplasty and sinus surgery; Surgical treatment of snoring/OSA; Knee surgery including arthroscopy, autografts/allografts, meniscal procedures and related implants; Shoulder procedures including shoulder arthroscopy; Spinal procedures including neurostimulator implantation/removal/revision.

ASC Site-of-Service Medical Necessity

Site of Service ASC considered medically necessary when ALL applicable site-of-service criteria in the full policy are met

Site-of-Service ASC criteria: Procedures listed as ASC-eligible (including shoulder arthroscopy codes and selected diagnostic/surgical procedures) must meet the policy's clinical and operational criteria to be considered medically necessary in an ASC setting.

Criteria include ASC accessibility within 30 miles, appropriate ASC equipment and physician privileges, and absence of patient-specific increased perioperative risk factors (see full policy for detailed list). Prior authorization/site-of-service review may be required per policy history.

The Site of Service Medical Necessity criteria within this policy does not apply to Indian Health Services (IHS) facilities. Providers should refer to the individual procedure medical necessity criteria rather than applying the site-of-service rules in this policy when the service is provided at an IHS facility.

Site-of-service review and the ASC site selection criteria described in this policy do not apply to Indian Health Services (IHS) facilities. This header-level exemption clarifies that IHS facilities are excluded from the site-of-service review requirements and related ASC placement determinations.

The policy identifies specific hospital-based sites of service that are considered not medically necessary for certain elective surgical procedures when the site-of-service criteria in this guideline are not met.
