Preferred therapies updated to list Epclusa, Harvoni, Mavyret, Sovaldi, and Vosevi as preferred for chronic HCV without or with compensated cirrhosis.
Genotyping and resistance-associated substitution (RAS) testing are specified as medically necessary when indicated by current guidelines.
Length of authorization and re-authorization statements: initial approvals may be up to 12 months; re-authorization considered investigational.
Added FDA approved uses for brand sofosbuvir-velpatasvir, brand ledipasvir-sofosbuvir, Sovaldi (sofosbuvir) and Zepatier (elbasvir-grazoprevir).
