Medicare NCD for allogeneic HSCT in multiple myeloma: Allogeneic HSCT is covered by Medicare only for beneficiaries with Durie-Salmon Stage II or III multiple myeloma, or ISS Stage II or III, and only when the beneficiary is participating in an approved prospective clinical study that meets CED criteria. Such studies must use appropriate statistical techniques to control for selection bias and confounding and assess outcomes including acute and chronic graft-vs-host disease, other transplant-related adverse events, and overall survival (quality of life optional).durie-salmon stage II/III or ISS stage II/III and participation in approved CED study

Allogeneic hematopoietic cell transplantation (allo-HCT), whether myeloablative or nonmyeloablative, when used as initial or salvage therapy for multiple myeloma is considered investigational by this policy and is not covered under the medical necessity criteria. The policy explicitly lists allo-HCT as investigational for both newly diagnosed and relapsed/salvage treatment settings for multiple myeloma.

The evidence does not support routine use of allogeneic HCT as first-line therapy in newly diagnosed multiple myeloma. Nonrandomized studies and consensus statements report heterogeneous patient populations and variable conditioning regimens; although reduced-intensity (nonmyeloablative) approaches lower transplant-related mortality, they are associated with higher relapse risk and there is no convincing evidence that allo-HCT improves overall survival compared with autologous HCT.

This medical policy applies to Premera Bluecross products and does not apply to Medicare Advantage plans. Member contract benefits should be confirmed separately using the member benefit booklet or by contacting member services.

Allogeneic HCT, including both myeloablative and nonmyeloablative (reduced-intensity) conditioning regimens, is considered investigational when used as either initial therapy for newly diagnosed multiple myeloma or as salvage therapy for relapsed disease; such use is not supported as medically necessary under this policy.

Nonmyeloablative or myeloablative allogeneic HCT as first-line therapy for multiple myeloma is not supported by convincing evidence to improve survival compared with autologous HCT. Limitations in available studies (heterogeneous populations, variable protocols, and inconsistent outcome reporting) prevent a conclusion that allo-HCT should be used routinely as first-line treatment.

Medicare has a National Coverage Determination (Coverage with Evidence Development) that limits Medicare coverage of allogeneic HSCT for multiple myeloma to beneficiaries with Durie-Salmon Stage II or III or ISS Stage II or III who are participating in an approved prospective clinical study meeting CED criteria (including appropriate methods to control for selection bias and assessment of outcomes such as acute and chronic GVHD, other transplant-related adverse events, and overall survival). Allogeneic HSCT performed outside these CMS CED requirements would not meet Medicare's coverage conditions for traditional Medicare beneficiaries.

For autologous HCT, authorization is expected after completion of induction therapy when clinical criteria are met. For tandem autologous HCT, authorization is conditional: tandem transplant may be considered medically necessary for individuals who do not achieve at least a near-complete or very good partial response after the first transplant or for select high‑risk individuals as described in guidelines.

Background

Multiple myeloma is a systemic malignancy of plasma cells and represents a substantial proportion of hematologic cancers. High-dose chemotherapy followed by hematopoietic cell transplantation is a treatment approach used in some patients. Autologous HCT is commonly used as consolidation after induction therapy in transplant-eligible individuals and has evidence of benefit for overall and progression-free survival in that setting. POEMS syndrome is a rare plasma cell disorder for which autologous HCT may be considered in disseminated disease; evidence for POEMS derives mainly from case series and cohort studies.
