Coverage Criteria — Admission, Continued Stay, and Not Medically Necessary

Admission — Medical Necessity

Skilled nursing facility (SNF) admission may be considered medically necessary when ALL of the following criteria are met:

Service and clinical appropriateness: Service requirements and clinical appropriateness described in policy are met (interdisciplinary 24-hour skilled services, documented goals, availability of consultative/diagnostic services, evaluation by transferring/discharging provider and SNF clinician, daily documentation of skilled services with weekly progress).

Admission qualifying conditions: Admission meets ONE of the following: (1) Skilled nursing services and medical treatment goals are described for the individual's current clinical status; (2) Complex wound care is described for the individual's current wound status (e.g., stage 3 or 4, or multiple stage 2 wounds requiring aseptic daily dressing changes); (3) Impaired functional condition needing rehabilitation where ALL of the following are described: current level of function, rehabilitation potential and realistic functional goals, AND an acute functional impairment requiring at least minimum assistance or mobility impairment (walk or wheelchair < 70 feet) AND one or more skilled nursing services or skilled therapies are provided at least daily; (4) Other skilled needs such as IV/IM injections given at least every 12 hours (not manageable at lower level of care); parenteral (TPN) or enteral feeding requiring intervention/management of complications; active management of a complex medication regimen with documented monitoring; ostomy complication requiring intervention and training not manageable in alternate setting; device or drain management (initial care of urinary or wound drains including bladder irrigation, nephrostomy, suprapubic catheter, JP/biliary drains); ventilator or tracheostomy weaning; new respiratory treatment or new use of oxygen or nasopharyngeal/tracheal suctioning to stabilize an acute condition.

Continued Stay — Medical Necessity

SNF continued stay may be considered medically necessary when ALL the following criteria are met:

Service and clinical appropriateness: Service requirements and clinical appropriateness described for admission continue to be met (ongoing interdisciplinary skilled services, documentation of skilled care and weekly progress, evaluations as appropriate).

Continued stay qualifying conditions: Continued stay meets ONE of the following: demonstrated clinical improvement with ongoing skilled nursing need; measurable and sustained functional progress with ongoing restorative therapy need; OR medical instability that affects participation or progression where the needed clinical interventions are described. AND one or more skilled nursing or skilled therapy services are provided at least daily.

Skilled therapy intensity: When treating an acute functional decline, therapy services must include one or more modalities provided for at least 1 hour per day, 5 days per week; the individual must be able and willing to actively participate and demonstrate rehabilitation potential.

Not Medically Necessary / Transition

Situations when SNF admission or continued stay is not medically necessary or when transition is appropriate:

Not medically necessary: Admission or continued stay is NOT medically necessary when skilled services can be managed at a lower level of care; no further clinical or functional progress is expected or achieved within a reasonable time; services are primarily custodial or maintenance in nature; the individual is unwilling or unable to participate in a therapeutic program; or services are routine/stable care needs (e.g., routine medication administration, routine indwelling bladder catheters, established ostomy/gastrostomy feedings, routine tracheostomy site care, oxygen) for a medically stable individual without other skilled needs.

Transition to lower level of care: Transition from SNF to an alternate level of care may be appropriate when ALL of the following are met: the individual is clinically stable; ongoing skilled nursing services can be safely provided in the home with home health or outpatient care; no signs of infection or stable on an anti-infective regimen that can be administered outpatient; adequate nutritional program that can be managed at home (e.g., home infusion or enteral feeding manageable at home); pain management adequate without frequent medication changes; neurologic status/mentation stable at baseline; AND if in SNF primarily for rehabilitation, further progress toward goals is not expected or can be achieved at a lower level of care OR the individual is no longer willing or able to participate in therapy.

History/Change log

Services that are primarily custodial or maintenance in nature are not considered medically necessary for SNF-level care when no other skilled needs exist. Examples include routine medication administration for medically stable individuals, routine indwelling bladder catheter care, established ostomy or gastrostomy tube feeding without complication, routine tracheostomy site care, oxygen therapy for a medically stable individual, and care that is limited to maintaining activities of daily living. Such routine, stable care needs do not meet the policy's skilled-service requirements and are excluded from coverage at the SNF level.

This medical policy does not apply to Medicare Advantage. Member contracts and benefit rules for Medicare Advantage plans differ; providers and members should consult the member benefit booklet or contact a customer service representative for plan-specific coverage and prior authorization requirements.

Admission or continued stay in an SNF is not medically necessary when the individual's skilled needs can be managed at a lower level of care, no further clinical or functional progress is expected or achieved within a reasonable period, services are primarily custodial/maintenance in nature, or the individual is unwilling or unable to participate in a therapeutic treatment program. Routine, stable care (for example routine IV/IM administration for medically stable patients, established enteral feedings without complications, or routine tracheostomy site care) without other skilled needs does not meet SNF medical necessity criteria.

Specific qualifying conditions: One of: described skilled nursing services and medical goals; complex wound care (stage 3/4 or multiple stage 2) requiring aseptic daily dressing changes; impaired functional condition needing rehabilitation with acute functional impairment (<70 feet mobility) and at least one skilled nursing/therapy service provided daily; or other listed skilled needs such as IV/IM injections every 12 hours, parenteral/enteral feeding with complications, complex medication management with monitoring, ostomy complication management/training, device/drain management, ventilator/tracheostomy weaning, or new respiratory treatments/suctioning.

Discharge / Transition Criteria

Discharge from a skilled nursing facility may be appropriate when the individual is clinically stable and ongoing skilled nursing needs can be safely provided in the home or outpatient setting. Additional considerations supporting transition include absence of active infection (or stability on an outpatient-administerable anti-infective regimen), ability to manage nutrition at home (including home infusion or enteral feeding arrangements), adequate and stable pain control, and neurologic/mental status at baseline and appropriate for the clinical condition.

When further progress toward rehabilitation goals is not expected or can be achieved at a lower level of care, or when the individual is no longer willing or able to participate in therapy, discharge from SNF-level care should be considered. The policy was updated to add and clarify admission and continued-stay conditions while maintaining the same intent for medically necessary skilled nursing and therapy services.
