Coverage Criteria — Reconstructive versus Cosmetic Services

Reconstructive/Medical Necessity

Covered when ALL of the following are met

General reconstructive coverage rule: Procedure corrects abnormal structures caused by congenital defects, developmental abnormalities, trauma, infection, tumors, or disease AND is performed to improve or restore bodily function when an objective physical functional impairment is present.

Procedure-specific reconstructive criteria

Covered when ALL of the following are met (procedure-specific examples)

Canthoplasty/canthopexy: May be medically necessary to correct epiphora (excessive tearing), desiccation of the corneal epithelium (corneal dryness), or corneal ulceration.

Chin surgery (genioplasty/mentoplasty): May be medically necessary for repair of maxilla or mandible resulting from trauma, injury, or disease.

Labiaplasty (reconstructive indications): May be medically necessary for any of the following: chronic irritation causing pain from friction during physical or sexual activity that has persisted >= 12 weeks despite conservative management; correction of atypical genitalia; repair of congenital asymmetrical labial growth in the presence of a functional impairment; or repair of a congenital defect (e.g., from congenital adrenal hyperplasia).12 weeks conservative management for chronic irritation

Conservative measures include loose-fitting underwear/clothing, topical emollients/ointments, protective padding, and hygiene

Otoplasty/Pinnaplasty (reconstructive): May be medically necessary when ears are absent or deformed from congenital defect (e.g., aural atresia, microtia, anotia), trauma, or disease and the procedure is performed to improve hearing by directing sound into the ear canal.

Nonsurgical infant ear molding (e.g., EarWell) may be medically necessary for infants 3 months of age or less

Scar revision (reconstructive): May be medically necessary when the scar causes symptoms or functional impairment (e.g., pain, contracture, skin tension, or restriction of joint movement) AND the scar resulted from accidental injury, trauma, burn, or a medically necessary surgical procedure.

Cosmetic (Not Medically Necessary)

Not covered when ALL of the following are true

Cosmetic exclusion general: Service is primarily to preserve or improve appearance (reshaping normal structures) and there is no physical functional impairment.

Examples enumerated in policy include abdominoplasty, breast augmentation or mastopexy for appearance, lip augmentation, liposuction for body contouring, injectable dermal fillers and many pharmaceutical agents and topical agents when used primarily for appearance

General coverage determination

Coverage is based on contract language and determination whether services meet reconstructive (medically necessary) criteria:

Core determination

Functional impairment assessment: Determine whether the service is primarily indicated to improve or correct a functional impairment versus primarily to improve appearance.
The presence or absence of a functional impairment is a critical point in interpreting coverage eligibility; may be straightforward for musculoskeletal conditions and more challenging for dermatologic conditions.
Etiology assessment: If no functional impairment is present, determine the etiology (congenital anomaly, trauma, post-therapeutic, disease) and review contract language to see if that etiology is included in the definition of reconstructive services.

Procedures and pharmaceuticals that are performed or used primarily for cosmetic purposes are excluded from coverage. The policy lists extensive examples of excluded surgical procedures (e.g., abdominoplasty, brachioplasty (arm lift), breast augmentation, mastopexy (breast lift), liposuction, lipectomy, lower body lift, neck tuck/cervicoplasty, rhinoplasty for aesthetic purposes, and many forms of facial rejuvenation), as well as injectable agents and topical products when their primary purpose is to improve appearance rather than to treat a documented functional impairment. The pharmaceutical examples include commonly used cosmetic agents such as Botox Cosmetic, Juvederm, Restylane, Radiesse, Sculptra, and other listed fillers and topical agents not FDA‑approved for a functional indication.

Coverage decisions depend on the member's contract in effect at the time of service; most plans do not cover services, drugs, or supplies for cosmetic purposes. Verify individual member benefits and applicable contract language prior to treatment because plan terms, exclusions, and limitations vary and will determine whether a service that could be reconstructive under clinical criteria is eligible for coverage.

Specific procedures called out in the policy as cosmetic (when performed to improve appearance without a documented functional impairment) include, but are not limited to: abdominoplasty (tummy tuck), arm lift (brachioplasty), breast augmentation, breast lift (mastopexy), buttock or thigh lift, canthopexy/canthoplasty for appearance, chin implants/reduction for appearance, dermabrasion, diastasis recti repair for appearance, facial rejuvenation and injectable fillers, inverted nipple correction for cosmetic reasons, labiaplasty for appearance, laser skin resurfacing, lip augmentation, liposuction for body contouring, lipectomy, neck tuck/lift, penis enhancement, otoplasty for appearance, rhytidectomy (face lift), scar revision for appearance, skin tag removal for appearance, tattoo removal for appearance, torsoplasty, and treatments for spider veins or wrinkles. The policy also enumerates many injectable and topical pharmaceutical agents considered cosmetic when used for appearance (examples: Daxxify, Jeuveau, Juvederm, Kybella, Radiesse, Restylane, Sculptra, Xeomin, and others).

Genital procedures described as 'vaginal rejuvenation' and related designer genital procedures (e.g., clitoral reduction, hymenoplasty, G‑spot amplification, vaginal tightening, and labiaplasty performed for aesthetic reasons) are generally considered cosmetic. The policy notes that labiaplasty and similar procedures have not been well studied and that professional guidance (ACOG) states these procedures are not medically indicated and lack documented long‑term safety and effectiveness. Additionally, the FDA has warned that the use of energy‑based devices for vaginal cosmetic procedures may be associated with serious adverse events and their safety and effectiveness for these indications has not been established.

Background

Reconstructive surgery is intended to correct abnormal structures caused by congenital defects, developmental abnormalities, trauma, infection, tumors, or disease and is generally performed to restore or improve bodily function when an objective physical functional impairment is present.
