Generic deferasirox — transfusional iron overload

Covered when ALL of the following are met for generic deferasirox in transfusional hemosiderosis

Deferasirox (generic) - Transfusional hemosiderosis: The individual is receiving blood transfusions at regular intervals AND prior to starting deferasirox the individual's serum ferritin level is greater than 1,000 micrograms/liter AND deferasirox is prescribed by or in consultation with a hematologistserum ferritin >1000 mcg/L

Generic deferasirox — NTDT

Covered when ALL of the following are met for generic deferasirox in non-transfusion dependent thalassemia (NTDT)

Deferasirox (generic) - NTDT: The individual is aged 10 years or older AND prior to starting deferasirox the individual's serum ferritin level is greater than 300 micrograms/liter AND deferasirox is prescribed by or in consultation with a hematologistage >=10; serum ferritin >300 mcg/L

Deferiprone — transfusional thalassemia

Covered when ALL of the following are met for generic deferiprone (Ferriprox/generic) in transfusional iron overload due to thalassemia syndromes

Deferiprone (generic/Ferriprox) - transfusional thalassemia: The individual is aged 3 years or older AND prior to starting deferiprone the individual's serum ferritin level was greater than 2,500 micrograms/liter AND deferiprone is prescribed by or in consultation with a hematologistage >=3; serum ferritin >2500 mcg/L

For branded Ferriprox, trial/inadequate response or intolerance to generic deferiprone after 3 months is required

Branded deferasirox — transfusional iron overload

Covered when ALL of the following are met for branded deferasirox formulations (Exjade, Jadenu, Jadenu Sprinkle) for transfusional hemosiderosis

Deferasirox for NTDT (evidence basis): Study 5 randomized NTDT patients with LIC ≥5 mg Fe/g dw and ferritin >300 mcg/L showed significant reduction in LIC at 52 weeks for deferasirox 5 and 10 mg/kg/day vs placebo; dose effect favoring 10 mg/kg/day.LIC ≥5 mg Fe/g dw; ferritin >300 mcg/L

All other uses of the medications listed in this policy that are not specifically described as covered (for example, indications outside transfusional hemosiderosis or NTDT as defined in this policy) are considered investigational and are not supported by this coverage document.

This medical policy does not apply to Medicare Advantage. Benefit determination for Medicare Advantage members should be made using the applicable Medicare Advantage plan documents and guidance.

For branded formulations (Exjade, Jadenu, Jadenu Sprinkle for deferasirox; Ferriprox for deferiprone), coverage requires a documented trial and either an inadequate response after 3 months of the respective generic agent or documented intolerance to the generic product before the branded product will be considered for medical necessity review.

Serious adverse events are described in the safety data for these agents and require clinical management and consideration when assessing ongoing therapy. Deferiprone’s most serious reported reaction is agranulocytosis and warrants close monitoring or discontinuation as clinically indicated. Deferasirox trials reported common adverse events (abdominal pain, nausea, vomiting, diarrhea, rash) and dose‑related increases in serum creatinine; the pooled premarketing discontinuation rate in the first year was substantial. When severe AEs occur, clinical judgment should guide whether therapy is continued, modified, or stopped and documentation of monitoring and specialist involvement (hematology) should be provided.

Background

Chronic blood transfusions result in progressive iron accumulation because the body has no mechanism to excrete excess transfused iron. This transfusional iron overload leads to iron deposition in organs — particularly the liver, heart and endocrine organs — increasing the risk of organ dysfunction and death. Serum ferritin and liver iron concentration (LIC) are used to quantify iron burden; sustained very high ferritin (for example, >=2,500 mcg/L) has been associated with organ toxicity and increased mortality. Iron chelators (oral deferasirox and deferiprone) bind excess iron to promote excretion and reduce organ iron deposition, but their use requires specialist oversight and laboratory monitoring due to potential serious adverse effects.
