Background: Pulsed radiofrequency delivers short intermittent bursts of energy (typically twice per second) with quiet phases that allow electrode cooling so temperatures remain below the neurodestructive threshold (~<42°C). By permitting higher voltage outputs and stronger electrical fields without reaching ablative temperatures, PRF is theorized to modulate ion channels, neurotransmitters, postsynaptic receptors, synaptic function, and immune markers.

Evidence limitations: PRF has been used for many peripheral neuropathies, arthrogenic pain, trigger points, and radiculopathy, but despite anecdotal and observational reports suggesting clinical benefit, there is a lack of high-quality randomized controlled trials and long-term comparative data to establish efficacy and durability.

Evidence summary — quality: The body of evidence includes a small number of RCTs, nonrandomized controlled trials, prospective uncontrolled trials, retrospective studies, case series, and case reports.
