Policy Coverage Criteria for Radiofrequency Volumetric Tissue Reduction and Nasal Swell Body Reduction
Defines Premera Bluecross medical policy coverage position for radiofrequency volumetric tissue reduction (RFVTR) for internal nasal valve collapse and radiofrequency or cryoablation for nasal swell body reduction to treat nasal obstruction, including coding and evidence summary. Applies to evaluation of medical necessity for these procedures (not applicable to Medicare Advantage).
New policy approved April 8, 2025 and effective May 1, 2025 establishing investigational status for RFVTR for internal nasal valve collapse and RF/cryoablation for nasal swell body reduction.
Coverage Summary & Position
This policy defines the coverage position for radiofrequency volumetric tissue reduction (RFVTR) for internal nasal valve collapse and radiofrequency or cryoablation for nasal swell body reduction to treat nasal obstruction. Background: nasal obstruction can be caused by internal nasal valve collapse (NVC) or nasal swell bodies (NSB), and RF techniques (for example, the VivAer Stylus) have been proposed as less invasive alternatives to surgery. Evidence summary: small randomized trials and systematic reviews report short-term symptom improvements but are limited by small sample sizes, short follow-up, and potential confounding; evidence is insufficient to demonstrate a net health benefit. Coverage stance: not_covered_cosmetic. Effective date: 2025-05-01; Last review: 2025-04-08.
