Coverage and Medical Necessity Criteria

Primary total ankle arthroplasty - medically necessary

Covered when ALL of the following are met:

Primary TAA coverage: Patient is 19 years or older; documented diagnosis of osteoarthritis OR rheumatoid arthritis OR post-traumatic arthritis; disabling pain for at least 3 months AND functional disability of the ankle that interferes with activities of daily living (ADLs); imaging consistent with advanced or end-stage tibiotalar arthritis with one or more of the following: joint space narrowing, osteophytes, subchondral sclerosis, subchondral cysts, deformity and bone remodeling; documentation of three months of failed non-operative conservative management including one or more medications (non-steroidal anti-inflammatory drugs, acetaminophen, or antirheumatic drugs/DMARDs if appropriate) AND a trial of one or more physical measures (ankle-foot orthosis >= 12 weeks OR physical therapy >= 12 weeks).see text

Revision total ankle arthroplasty may be considered medically necessary when ANY of the following are present:

The policy designates prosthetic replacement of the talus used as part of total ankle arthroplasty or revision, or total ankle arthroplasty combined with a total talar prosthesis, as investigational. Examples listed include the Patient Specific Talus Spacer (3D-printed talus implant), Restor3d Talus Replacement, and the 4WEB Talar Replacement Device. These uses are not considered standard treatment under this policy and therefore are not covered.

Total ankle arthroplasty is a surgical procedure and is not itself regulated by the FDA; however, devices used in the procedure require FDA approval or clearance. The policy notes that several devices have received FDA approval or clearance (product codes HSN and NTG), and that certain talus replacement implants have been authorized under Humanitarian Device Exemptions (product code QNN), including HDEs granted in 2021, 2023, and 2024.

Total ankle arthroplasty is considered not medically necessary when any of the following exclusionary conditions are present: absence of the medial or lateral malleolus; active infection in the ankle joint or adjacent bones; avascular necrosis of the talus; Charcot joint of the affected ankle; significant hindfoot, forefoot, or knee malalignment; insufficient bone stock or insufficient ligament support; loss of musculature support in the limb; peripheral neuropathy or peripheral vascular disease affecting the target extremity; poor skin conditions from scars or trauma; severe ankle deformity precluding proper alignment; and severe osteoporosis or osteopenia that may affect bony fixation.

When the clinical and imaging findings submitted do not meet the policy's specified medical necessity criteria, services for total ankle arthroplasty may be considered not medically necessary and denied. Providers should document the required clinical indications, duration of symptoms, prior conservative management, and imaging consistent with the policy to support medical necessity.

Total ankle arthroplasty (TAA), also called ankle replacement, is a surgical procedure that replaces the diseased tibiotalar joint surfaces with artificial prosthetic components (typically metal and polyethylene) to relieve pain and preserve ankle motion in adults with end‑stage tibiotalar arthritis. TAA is an alternative to ankle arthrodesis; postoperative care commonly includes an initial period of limited weight bearing, rehabilitation beginning around four weeks, and implants that are generally expected to function for approximately 10 years.
