Anterior Cervical Fusion

Anterior cervical fusion considered medically necessary when specific diagnostic or clinical criteria are met

Listed primary indications: Degenerative cervical spondylosis; Infection of cervical spine; Ossification of the posterior longitudinal ligament (OPLL) at 1–3 levels associated with myelopathy; Posttraumatic cervical instability (e.g., unstable anterior column fracture); Spine fracture and/or dislocation; Tumor of the cervical spine causing pathologic fracture, cord compression, or instability

Any ONE of the listed conditions qualifies

For cervical radiculopathy: Unremitting radicular pain or progressive weakness secondary to nerve root compression

AND the additional criteria for surgery for radiculopathy must be met (see imaging and conservative therapy requirements)

For spondylotic myelopathy: Signs or symptoms of myelopathy (e.g., upper limb weakness beyond a single root, lower limb weakness, loss of dexterity, frequent falls, hyperreflexia, positive Hoffmann sign, spasticity, gait abnormality, positive Babinski) AND cervical spine MRI or CT with myelogram within the past 12 months demonstrating cord compression corresponding to symptoms due to herniated disk, osteophyte, or OPLLimaging ≤12 months

Posterior cervical fusion may be medically necessary for specified trauma, instability, congenital, infectious, neoplastic and failure conditions

Listed primary indications: Atlas and axis fractures; Bilateral locked facets; Cervical instability in individuals with Down syndrome; Cervical instability in skeletal dysplasia or connective tissue disorders; Disruption of posterior ligamentous structures; Facet fractures with dislocation; Implant/instrumentation failure demonstrated on imaging (malposition, dislocation/subluxation, vertebral body fracture, hardware breakage); Infection of the cervical spine requiring decompression/debridement; Klippel-Feil syndrome; OPLL without kyphosis with associated myelopathy; Tumor of the cervical spine causing pathologic fracture, cord compression, or instability; Part of stabilization with corpectomy/laminectomy at the cervicothoracic junction; Posttraumatic cervical instability; Subluxation and cord compression in rheumatoid arthritis

Multilevel spondylotic myelopathy: Signs or symptoms of myelopathy are present (see policy) AND imaging correlating to signs/symptoms within the past 12 monthsimaging ≤12 months

Cervical Discectomy

Cervical discectomy may be medically necessary for herniated disc when clinical and imaging criteria are met

Clinical signs: Signs and symptoms of radiculopathy and/or myelopathy (e.g., pain radiating into shoulder/arm/hand in a dermatomal pattern, numbness/tingling, motor weakness in a nerve root distribution, increased or abnormal reflexes corresponding to affected level, loss of sensation in a dermatomal pattern)

At least one listed sign/symptom required

Clinical presentations qualifying for surgery: Persistent debilitating neck, back, or arm pain unresponsive to at least 6 weeks of conservative nonsurgical therapy (including prescription-strength analgesics with anti-inflammatory medications, physical therapy, and evaluation/management of cognitive/behavioral/addiction issues) OR persistent or progressive myelopathy unresponsive to at least 6 weeks of conservative nonsurgical therapy≥6 weeks conservative therapy

Imaging correlation: Documentation of nerve root compression on MRI or CT at a level that corresponds with the individual's symptoms

Cervical Laminectomy

Cervical laminectomy may be medically necessary for fracture, infection, tumor, or spinal stenosis/myelopathy when specified criteria are met

Immediate indications: Spinal fracture, dislocation, locked facets, or displaced fracture fragment confirmed by imaging; Spinal infection confirmed by imaging; Spinal tumor confirmed by imaging

Any ONE of these immediate indications qualifies

For spinal stenosis or cord compression: Either rapidly progressing neurologic deficits OR persistent debilitating pain unresponsive to at least 6 weeks of conservative nonsurgical therapy (including prescription-strength analgesics with anti-inflammatories, physical therapy, and evaluation/management of cognitive/behavioral/addiction issues) OR signs/symptoms of cervical myelopathy (e.g., difficulty with fine hand movements, incoordination, thenar/hypothenar atrophy, diffuse hyperreflexia and bilateral Babinski responses, decreased vibration/proprioception at C5 or below, inability to tandem walk, bowel/bladder incontinence) AND MRI or other neuroimaging within the past 12 months demonstrates pathologic anatomy corresponding to signs/symptoms≥6 weeks conservative therapy; imaging ≤12 months

Indications supported by evidence: Any of the following may be supported by the evidence: (a) cervical herniated disc with progressive or refractory radiculopathy where discectomy improves short-term symptoms; (b) cervical spinal stenosis with cord or root compression where laminectomy provides benefit; (c) space-occupying lesions of the spinal canal where laminectomy may be necessary despite limited comparative evidence

Evidence strength varies; long-term equivalence to conservative care often observed by 12 months

Cervical discectomy is considered not medically necessary when the policy's required criteria are not met. Specifically, discectomy is not supported if there is not documentation of (1) signs or symptoms of radiculopathy or myelopathy, (2) failure of an adequate trial of conservative nonsurgical therapy (typically ≥ 6 weeks including prescription-strength analgesics with anti-inflammatories, physical therapy, and management of cognitive/behavioral/addiction issues when present), and (3) imaging (MRI or CT) demonstrating nerve root compression at a level that corresponds with the individual's symptoms.

This policy applies to the adult population only and is intended for individuals aged 19 years and older. Pediatric-specific considerations, diagnostic thresholds, and treatment pathways are excluded from the scope of this document and are not addressed in the policy statements or coverage criteria.

Non-emergent cervical fusion is not recommended as first-line therapy for chronic cervical pain. The policy requires that providers attempt a comprehensive course of nonoperative management (e.g., physical therapy, cervical immobilization, prescription analgesics including anti-inflammatories, and appropriate behavioral/addiction management) and document failure of conservative care prior to considering fusion for chronic pain. Short-term symptomatic improvements with fusion do not negate the expectation that conservative therapy be tried first because longer-term outcomes at 12 months are comparable to nonoperative care.

This medical policy does not apply to Medicare Advantage members. Providers and members should consult the member benefit booklet or contact customer service to determine coverage specifics for Medicare Advantage plans.

For elective cervical spine procedures, inpatient hospitalization is not medically necessary as the site of service unless the site-of-service criteria are met. The policy lists specific perioperative risk factors (for example, ASA class III or higher and other cardiac, pulmonary, hepatic, renal, or bleeding risks) that may justify inpatient care; requests for fusion procedures beyond two levels require medical director review.

Most cervical disc herniations tend to decrease in size and many patients experience clinical improvement over time. Because of this natural history, initial management is conservative—analgesics and activity modification with adjuncts as appropriate—and surgery without evidence of failed conservative management or without urgent neurologic indications is generally not supported.

Fusion performed solely for chronic axial neck pain without documentation of failed conservative therapy and without objective findings of neural compression on clinical exam and imaging is not supported. Evidence shows comparable long-term outcomes between fusion and conservative management by 12 months while fusion carries risks of complications; therefore objective neural compression and prior nonoperative treatment must be documented to justify fusion for chronic pain.
