Medical Necessity and Coverage Criteria

General elective procedure and site-of-service criteria

Applies to elective single-level cervical decompression and single-level cervical fusion in adults age 19 and older. Site of service must be medically necessary.

Population and scope: Age 19 or older AND single-level procedure

Requests for fusions >2 levels must be reviewed by a medical director

Accepted sites of service: Off‑campus outpatient hospital/medical center OR On‑campus outpatient hospital/medical center OR Ambulatory surgical center OR Inpatient hospital when the individual has clinical conditions that increase risk for complications

Inpatient site of service is medically necessary only for listed increased-risk conditions (examples: ASA III+, significant cardiovascular/liver/pulmonary/renal risk, morbid obesity BMI ≥50, pregnancy, bleeding disorders, anticipated transfusion, prolonged surgery >3 hours, substance dependence); this list may not be all inclusive

Anterior cervical fusion — covered indications

Anterior cervical fusion may be considered medically necessary when any ONE of the following conditions is present:

Anterior cervical fusion indications: Degenerative cervical spondylosis with kyphosis causing cord compression OR Infection of the cervical spine requiring decompression or debridement OR Ossification of the posterior longitudinal ligament (OPLL) at 1–3 levels associated with myelopathy OR Posttraumatic cervical instability (e.g., unstable anterior column fracture) OR Spine fracture and/or dislocation associated with mechanical instability (including locked facets or displaced fracture fragment) OR Tumor of the cervical spine causing pathologic fracture, cord compression, or instability

Any ONE of the listed conditions qualifies

Anterior fusion specific criteria

Anterior cervical fusion specific criteria for radiculopathy, spondylotic myelopathy, pseudarthrosis, and adjacent‑segment disease.

Radiculopathy indication for anterior fusion: Unremitting radicular pain or progressive weakness due to nerve root compression AND symptoms unresponsive to at least 6 weeks of conservative nonsurgical therapy including ALL of: prescription‑strength analgesics (including anti‑inflammatories and adjunctive agents as appropriate), physical therapy, and evaluation/management of cognitive/behavioral/addiction issues when present AND MRI or CT/myelogram within the past 12 months demonstrating spinal stenosis or nerve root compression at the level correlating with symptoms and physical exam>= 6 weeks

Conservative therapy components defined in policy; expedited surgery allowed for rapidly progressive neurologic deficits

Spondylotic myelopathy indication: Signs or symptoms of myelopathy (examples: upper limb weakness beyond a single nerve‑root distribution, lower limb weakness, loss of dexterity/clumsiness, bowel/bladder incontinence, frequent falls, hyperreflexia, Hoffmann sign, gait abnormality, increased extremity tone/spasticity, positive Babinski) AND MRI or CT/myelogram within the past 12 months demonstrating spinal cord compression corresponding to symptoms and exam due to herniated disk, osteophyte, or OPLL

Any ONE or more listed neurologic findings suffices when imaging correlates

Posterior cervical fusion criteria

Posterior cervical fusion may be considered medically necessary for any ONE of the listed indications and in specific multilevel or instability situations when criteria are met.

Posterior fusion indications: Atlas and axis fractures OR Bilateral locked facets OR Cervical instability in individuals with Down syndrome OR Cervical instability in skeletal dysplasia or connective tissue disorders OR Disruption of posterior ligamentous structures OR Facet fractures with dislocation OR Implant/instrumentation failure demonstrated on imaging OR Infection of the cervical spine requiring decompression/debridement OR Klippel‑Feil syndrome OR OPLL without kyphosis with associated myelopathy OR Part of stabilization with corpectomy/laminectomy at the cervicothoracic junction OR Part of stabilization with laminectomy (e.g., at C2) OR Posttraumatic cervical instability OR Subluxation and cord compression in rheumatoid arthritis OR Tumor causing pathologic fracture, cord compression, or instability

Any ONE of the listed conditions qualifies

Multilevel spondylotic myelopathy without kyphosis: Signs or symptoms of myelopathy are present (e.g., upper limb weakness beyond a single nerve‑root distribution, lower limb weakness, loss of dexterity) AND criteria for myelopathy per policy are documented

Posterior fusion may be considered when multilevel myelopathy without kyphosis criteria are met

Cervical discectomy criteria

Cervical discectomy (anterior open with or without fusion or minimally invasive posterior approaches) may be considered medically necessary when ALL criteria below are met.

Discectomy indication: Signs and symptoms of radiculopathy and/or myelopathy (examples: dermatomal radiating pain, numbness/tingling, motor weakness, abnormal reflexes) AND one of the following clinical presentations: rapidly progressing neurologic deficits OR persistent debilitating neck/arm pain unresponsive to at least 6 weeks of conservative nonsurgical therapy including ALL of: prescription‑strength analgesics, physical therapy, and evaluation/management of cognitive/behavioral/addiction issues when present OR persistent or progressive myelopathy unresponsive to at least 6 weeks of conservative nonsurgical therapy AND imaging (MRI or CT) documenting nerve root or cord compression at a level that corresponds with the individual's symptoms>= 6 weeks; imaging within 12 months

Cervical discectomy refers to open anterior cervical discectomy (with or without fusion) or minimally invasive posterior approaches

Discectomy not medically necessary: Cervical discectomy is not medically necessary for treatment of a herniated disc when the above criteria are not met

Surgery for disc herniation without documentation of persistent symptoms or failed conservative care is not supported

Cervical laminectomy criteria

Cervical laminectomy may be considered medically necessary for absolute indications or for stenosis/cord/root compression when criteria are met.

Absolute laminectomy indications: Spinal fracture, dislocation, locked facets, or displaced fracture fragment confirmed by imaging OR Spinal infection confirmed by imaging OR Spinal tumor confirmed by imaging

Any ONE of the listed conditions qualifies

Laminectomy for spinal stenosis or neural compression: At least ONE of: rapidly progressing neurologic deficits OR persistent debilitating pain unresponsive to at least 6 weeks of conservative nonsurgical therapy including ALL of: prescription‑strength analgesics, physical therapy, and evaluation/management of cognitive/behavioral/addiction issues when present OR signs/symptoms of cervical myelopathy (examples: difficulty with fine hand movements, incoordination, thenar/hypothenar atrophy, diffuse hyperreflexia, bilateral Babinski, decreased sensation/vibration/proprioception, inability to tandem walk, bowel/bladder incontinence) AND MRI or other neuroimaging within the past 12 months demonstrating pathologic anatomy corresponding to the individual's signs and symptoms>= 6 weeks; imaging within 12 months

For space‑occupying lesions evidence is limited but current standard of care supports laminectomy

Medical necessity coverage criteria

Covered when ALL of the following are met

Authorization and documentation: Prior authorization approval obtained and clinical information submitted to support medical necessity

See prior authorization and documentation sections for required materials (imaging, history, conservative therapy documentation)

Clinical evaluation: Clinical findings documented including the individual's stated symptoms, duration, and assessment of comorbid physical and psychological conditions

Include history of prior spine surgery where applicable

Conservative therapy: Trial and failure or contraindication to conservative nonoperative interventions (activity modification for at least 6 weeks; oral analgesics/anti‑inflammatories; physical therapy; chiropractic manipulation; epidural steroid injections) unless emergent or rapidly progressive neurologic deterioration>= 6 weeks for activity modification

Expedited surgery allowed for progressive neurologic deficit

Medical necessity criteria for procedures

Covered when clinical and diagnostic findings align with evidence‑based indications:

Cervical fusion for chronic pain: Non‑emergent cervical fusion may be considered only after comprehensive nonoperative therapy has failed AND physical examination and diagnostic imaging demonstrate neural compression at the appropriate level

Literature shows faster short‑term relief with fusion but comparable outcomes at 12 months; weigh benefits versus higher complication risks

Cervical discectomy for herniated disc with radiculopathy: Cervical discectomy is supported for patients with herniated cervical disc and radiculopathy that is rapidly progressing or refractory to conservative care; evidence demonstrates improved short‑term symptoms though long‑term outcomes converge with nonoperative care

Evidence includes small RCTs and systematic reviews; extrapolation from lumbar discectomy informs benefit

Cervical laminectomy for stenosis or neural compression: Cervical laminectomy is supported for patients with spinal stenosis and spinal cord or nerve root compression when clinical and imaging findings correspond; evidence parallels lumbar decompression literature

Cervical discectomy (open anterior discectomy with or without fusion, or minimally invasive posterior approaches) is not medically necessary when the policy's discectomy criteria are not met. The criteria require documented signs or symptoms of radiculopathy and/or myelopathy and either rapidly progressing neurologic deficits, or persistent debilitating pain unresponsive to ≥6 weeks of comprehensive conservative nonsurgical therapy, or persistent/progressive myelopathy unresponsive to ≥6 weeks of conservative care, together with imaging (MRI or CT) demonstrating nerve root compression at the level corresponding to symptoms. If these elements are absent, coverage for discectomy is not supported.

For elective single-level cervical decompression and fusion procedures in adults, inpatient hospital or medical center admission is not medically necessary when the policy's inpatient risk criteria are not met. Preferred medically necessary sites of service are outpatient settings (off‑campus or on‑campus outpatient hospital/medical center or ambulatory surgical center); inpatient care is reserved only for individuals with increased perioperative risk (for example, ASA class III+, significant cardiovascular, pulmonary, hepatic, or renal disease, morbid obesity with BMI ≥50, pregnancy, bleeding disorders, prolonged anticipated surgery, or other listed high‑risk conditions).

This policy applies to the adult population only and is intended for individuals aged 19 years or older. Pediatric or other age‑specific guidance is not provided in this document; management decisions for patients younger than 19 should be guided by applicable pediatric policies and clinical judgment.

Evidence for cervical laminectomy to treat space‑occupying lesions of the spinal canal comes largely from case series and is limited by small, retrospective, uncontrolled studies. Although most series report symptomatic improvement or arrest of progression for patients with myelopathy, the evidence is insufficient for comparative efficacy versus alternatives. Nonetheless, current standard of care, clinical input, and guideline consensus support laminectomy as an appropriate intervention in these situations, and the policy considers laminectomy medically necessary for space‑occupying lesions when clinically indicated.

This medical policy does not apply to Medicare Advantage. Member benefits and applicability to Medicare Advantage plans should be confirmed through the member's contract or plan documents.

When the specific medical necessity criteria for cervical discectomy are not fulfilled — including documented radiculopathy/myelopathy, appropriate imaging showing corresponding nerve root compression, or failure of ≥6 weeks of comprehensive conservative therapy when indicated — cervical discectomy is considered not medically necessary and is not supported for coverage.

Surgery for cervical disc herniation without documentation that symptoms are persistent or progressively worsening, or without clear documentation of failure of conservative nonsurgical care, is not supported. Most disc herniations tend to improve over time; therefore operative intervention should be reserved for patients with persistent debilitating symptoms or rapidly progressive/acute neurologic deficits, or when conservative management has failed as defined in the policy.

Elective cervical fusion performed for chronic axial neck pain alone — without prior comprehensive nonoperative management and without objective physical examination or imaging evidence of neural compression — is not supported. Systematic evidence finds that fusion may provide faster short‑term symptom relief but outcomes are comparable to conservative care by 12 months, while fusion carries higher rates of complications; therefore fusion should be reserved for patients who have failed comprehensive conservative therapy and who have objective findings consistent with neural compression at the appropriate level.

Prior authorization review and approval is required for all indications of cervical spine surgery, including cervical discectomy, laminectomy, and fusion. Submit complete supporting clinical information at the time of request, including recent neuroimaging and documentation of conservative care where applicable.

Denial Risk

Site‑of‑Service Review and Denial Risk

Certain elective single‑level cervical decompression and single‑level cervical fusion procedures are subject to site‑of‑service review. Inpatient hospitalization for elective procedures is not medically necessary unless inpatient site‑of‑service criteria are met (e.g., significant anesthesia, cardiovascular, pulmonary, renal, hepatic, bleeding risk, pregnancy, morbid obesity, or other conditions increasing perioperative risk).

Clinical Background and Evidence Context

Cervical fusion stabilizes vertebral segments to address instability or decompress neural elements via accompanying procedures. Laminectomy or laminotomy remove part or all of the lamina to decompress the spinal cord or nerve roots and may be needed when disc removal alone is insufficient. Discectomy removes herniated disc material compressing nerve roots. These procedures are used for indications that include radiculopathy, myelopathy, fracture, infection, tumor, ossification of the posterior longitudinal ligament (OPLL), and mechanical instability; the choice among decompression, fusion, or combined approaches depends on the specific pathology, neurologic findings, and radiographic evidence.
