inv-01: Carpal tunnel release (endoscopic or open) — Medically necessary

Covered when ALL of the following are met

Diagnostic confirmation: Documentation supports a clinical diagnosis of carpal tunnel syndrome by EITHER electrodiagnostic testing confirming carpal tunnel syndrome OR a CTS-6 score > 12.CTS-6 > 12 or positive electrodiagnostic testing

inv-03: Investigational / Not covered techniques

The following surgical techniques are considered investigational for the treatment of carpal tunnel syndrome

Investigational techniques: Thread carpal tunnel release (TCTR) OR ultrasound-guided percutaneous needle release (PCTR) OR ultrasound-guided percutaneous intracarpal tunnel balloon dilation release are considered investigational and not medically necessary.

See Appendix and Evidence Review for discussion of limited/small studies; intracarpal tunnel balloon dilation release was added to investigational techniques.

Carpal tunnel release (open or endoscopic) is considered not medically necessary when the required medical necessity criteria are not met. Specifically, coverage is denied if documentation does not support a clinical diagnosis of carpal tunnel syndrome by either confirmatory electrodiagnostic testing (e.g., nerve conduction study or electromyography) or a CTS-6 score greater than 12, or if there is no documentation of failure of at least 6 weeks of conservative treatment (night hand immobilization and/or corticosteroid injection), unless severe neurologic findings (for example, thenar wasting or impaired two-point discrimination on the radial side) are present.

Ultrasound-guided percutaneous intracarpal tunnel balloon dilation release is listed as investigational and therefore is not considered medically necessary/covered. This technique was added to the policy’s investigational procedures list and is explicitly excluded from coverage.

Other minimally invasive techniques remain investigational and are not covered, including thread carpal tunnel release (TCTR) and ultrasound-guided percutaneous needle release (PCTR). These approaches are identified in the policy as investigational for treatment of carpal tunnel syndrome.

The policy explicitly identifies the following surgical techniques as investigational and not medically necessary: thread carpal tunnel release (TCTR), ultrasound-guided percutaneous needle release (PCTR), and ultrasound-guided percutaneous intracarpal tunnel balloon dilation release. Requests for these procedures are not eligible for coverage under this policy.

For surgical CPT codes (including but not limited to endoscopic and open carpal tunnel release), prior authorization will only be considered when documentation demonstrates that the individual meets medical necessity criteria. For primary surgery this includes diagnostic confirmation and evidence of failed conservative therapy unless exempted by severe findings.

Document either electrodiagnostic testing confirming carpal tunnel syndrome OR a CTS‑6 score > 12.
Document at least 6 weeks of conservative therapy (see conservative therapy callout) unless there is thenar wasting, impaired two‑point discrimination on the radial thumb side, or other severe carpal tunnel findings.

Background — Clinical Context

Carpal tunnel syndrome is compression of the median nerve at the wrist producing numbness, tingling, pain, and weakness in the thumb, index, middle fingers and the radial side of the ring finger. Symptoms can be progressive and are commonly related to repetitive activities or systemic conditions. Initial management is conservative (night splinting and/or corticosteroid injection); when conservative measures fail and diagnostic criteria are met, surgical release of the transverse carpal ligament (open or endoscopic carpal tunnel release) may be considered.

Phalen's test

DefinitionA wrist-flexion maneuver held 30–60 seconds in which the patient presses the backs of the hands together to flex the wrists; positive if it reproduces numbness, tingling, or pain in the median nerve distribution (thumb, index, middle, and thumb-side of ring finger).

Insufficient Diagnostic or Conservative Treatment Documentation — Denial Risk

If the submitted medical record does not document required diagnostic testing or CTS‑6 scoring and/or does not document the requisite conservative therapy (or a documented exception), the request is at risk for denial.

Absence of electrodiagnostic test results and no CTS‑6 score recorded increases denial risk.
Lack of documentation of ≥6 weeks of conservative care (night splinting and/or corticosteroid injection) — or absence of documented reason why conservative therapy was not required — may result in denial.
