Background & Evidence

Limb compression devices (intermittent pneumatic compression, IPC) reduce postoperative VTE risk and are widely used in-hospital; pharmacologic prophylaxis is the preferred approach in most surgical patients per national guidelines (ACCP, ACOG, ASH, AAOS). Mechanical prophylaxis is recommended when bleeding risk is high or when pharmacologic agents are contraindicated.

Evidence supports IPC use in the hospital setting and meta-analyses indicate benefit when IPC is added to pharmacologic prophylaxis during hospitalization, but there are no RCTs establishing incremental benefit of home IPC as an adjunct to anticoagulation after discharge.
