Fenfluramine / Cardiac Risk Background

Fenfluramine (Fintepla) has historical safety concerns related to valvulopathy and pulmonary arterial hypertension when used at higher anorexiant doses and/or with agents that increase serotonergic activity. Current trials exclude patients with cardiovascular disease and monitor for valvular heart disease and PAH. Prescribers should be aware of these risks and the exclusion/monitoring practices used in trials.

Trials excluded patients with pulmonary arterial hypertension or cardiovascular disease.
Dose limits in seizure indications are substantially lower than historic anorexiant regimens.
Concomitant use with MAOIs or other serotonergic agents may increase risk.

Clobazam (Sympazan, Onfi, oral suspension, or tablet) may be considered medically necessary for treatment of seizures associated with Lennox-Gastaut syndrome when all of the following are met:

- Has tried generic clobazam tablet or clobazam suspension first and had an inadequate response or intolerance to generic clobazam tablet or suspension.
- Has tried and had an inadequate response or intolerance to at least 1 additional generic anti-seizure medication.

Clinical Background and Evidence

Seizures result from abnormal electrical activity in the brain and can be isolated events or part of an epilepsy disorder characterized by recurrent unprovoked seizures. Seizure types include generalized seizures, which involve large areas of the brain, and focal seizures, which originate in a specific region. Treatment selection depends on seizure type, patient age, prior treatment response, and comorbidities; options include antiepileptic drugs, diet, surgery, and devices. The criteria in this policy describe when specific antiseizure medications may be considered medically necessary based on those clinical factors.

inv cu-31: Fintepla (fenfluramine) — clinical evidence summary (trial summaries; informational)

Fintepla trial participants: Phase 3 trials randomized 206 children and young adults with Dravet syndrome; trials excluded patients with PAH or cardiovascular disease.

Fenfluramine has a history of cardiovascular risk when used at anorexiant-level doses and in combination with other appetite suppressants. Reports from the weight-loss era (typical doses of 20–120 mg/day) documented valvular heart disease and pulmonary hypertension, and metabolites such as norfenfluramine have been implicated via 5HT2B receptor activation. For antiseizure use, the policy and clinical trials explicitly exclude patients with prior valvular heart disease or pulmonary arterial hypertension and use lower, indication-specific dosing; historical high-dose anorexiant use is therefore considered outside the approved/covered dosing context.

Has tried a generic phenytoin product first and had an inadequate response or intolerance to generic phenytoin.
Has tried and had an inadequate response or intolerance to at least 1 additional generic anti-seizure medication.
Dose is limited to 625 mg per day.

Elepsia XR (levetiracetam extended-release) may be considered medically necessary for treatment of partial-onset seizures in individuals aged 12 years and older when all the following are met:

ALL of the following

Has tried generic levetiracetam extended-release and had an inadequate response or intolerance to generic levetiracetam extended-release.
Has tried and had an inadequate response or intolerance to at least 1 additional generic anti-seizure medication.
Dose is limited to 3,000 mg per day.
