Skin biopsy with IENF density assessment is a validated, reproducible marker of small fiber sensory pathology and may be considered for diagnosis of distal symmetric polyneuropathy (DSP), particularly small fiber sensory neuropathy (SFSN) (England et al., 2009b; Level C/Class III).
Skin biopsy should be performed in patients with painful/burning feet of unknown origin and clinical impression of small fiber dysfunction (EFNS/PNS grade B).
Skin biopsy with appropriate histological processing and image analysis should be performed in patients with clinical signs of small fiber dysfunction to determine IENF density (NeuPSIG level B).
Measurement of IENF density may be used in follow-up and to detect treatment response in diabetic patients with small fiber neuropathy (NeuPSIG level C).

Background

Skin biopsy for nerve fiber density testing is performed using a small (typically 3 mm) punch biopsy; tissue sections are stained with an antibody to protein gene product 9.5 (PGP 9.5)

Clinical utility described in the policy includes diagnosis and monitoring of small fiber sensory neuropathy (SFSN) and distal symmetric polyneuropathy (DSP), assessment of autonomic (sudomotor) innervation (research and select clinical contexts), and use in multiple disease-specific settings (diabetes, Fabry disease, sarcoidosis, Parkinson disease, CMT1A, Ehlers–Danlos, Friedreich’s ataxia, fibromyalgia, ganglionopathy differentiation). National and specialty guidance (AAN/AANEM/AAPM&R, EFNS/PNS, NeuPSIG, IMMPACT) recognize skin biopsy with IENF density assessment as a validated technique with defined roles in selected patients.

Evidence summaries report diagnostic performance from guideline reviews and studies: case–control studies showed sensitivity for decreased IENF density ranging approximately from 45% to 90% and specificity of about 95%–97% for excluding polyneuropathy (England et al., 2009b).

For selected symptomatic patients at the dorsal foot, an IENF density threshold of 8 fibers/mm provided a reported sensitivity 88% and specificity 91% for small fiber sensory neuropathy (England et al., 2009b).

Spatial distribution metrics can improve diagnostic yield: an interfiber distance (a stretch of denervated epidermis) of 350 µm was identified as a discriminator with reported AUC 0.85, sensitivity 74%, specificity 94% (Piscosquito et al., 2021).
