Defines medical necessity criteria, site-of-service rules, documentation requirements, and coding for single-level cervical decompression surgeries and single-level cervical fusions in adults (age ≥19). Prior authorization required for all indicated cervical spine surgeries. Smoking within 6 weeks before elective fusion is a contraindication.
Title changed and policy expanded to include posterior cervical fusion, cervical discectomy, and cervical laminectomy (11/01/18).
