Gastrointestinal panel appropriate use: Order GIP testing for patients with diarrhea when clinical features (fever, bloody/mucoid stools, severe cramping, symptoms >7 days) or public health/outbreak concerns exist; NAATs can be used as adjuncts to traditional methods and repeat testing within seven days is not considered medically necessary.

Policy scope and history

The policy explicitly excludes molecular detection-based panel testing of urine pathogens for the diagnosis of urinary tract infections and molecular panel testing to screen for or diagnose wound infections from reimbursement because there is insufficient published evidence demonstrating clinical benefit (e.g., GENETWORx UTI and wound tests are cited as examples).

Multiplex PCR CSF (meningitis/encephalitis) panels should be ordered only when the patient has a moderate-to-high pretest probability of CNS infection based on clinical presentation and supporting CSF findings (cell count, glucose, protein). Ordering these panels in patients without supportive CSF abnormalities increases the likelihood of false positives and is discouraged.

Molecular multiplex panels do not replace bacterial and fungal culture. Culture and antimicrobial susceptibility testing remain essential for guiding therapy, epidemiologic surveillance, vaccine-related decisions, and local antibiograms; panels are adjuncts rather than substitutes.

The National Institute for Health and Care Excellence (NICE) concluded there is insufficient evidence to recommend routine adoption of integrated multiplex PCR gastrointestinal pathogen panels (for example, xTAG, FilmArray GI, Faecal Pathogens B assay) for suspected gastroenteritis and called for additional data on clinical utility.

The policy contains an explicit exclusion: testing of urine or wound pathogens using molecular panel methods is not reimbursable due to lack of supportive published literature demonstrating clinical benefit (the policy lists urine and wound panel examples).

Use of molecular panel testing for urine-based UTI diagnosis and for wound infection panels is considered not reimbursable (not medically necessary) as stated in the policy; specific proprietary tests are cited as examples.

Routine use of multiplex PCR meningitis/encephalitis panels as a screening test in patients with low pretest probability or without CSF abnormalities is not appropriate. The policy emphasizes that molecular testing is most useful when CSF analysis and clinical presentation indicate moderate-to-high likelihood of CNS infection.

Routine multiplex CNS panel testing ordered without clinical or CSF evidence of central nervous system infection (i.e., low pretest probability) is not covered because it increases false positives and is not considered appropriate testing.

Routine adoption of integrated multiplex PCR gastrointestinal panels is not covered as recommended by NICE, which found insufficient evidence to support routine implementation in the NHS and called for more data on clinical utility.
