Pathogen Panel Testing (Multiplex / Molecular)
Outpatient coverage and reimbursement policy for multiplex molecular pathogen panel testing (gastrointestinal, respiratory, CSF, blood, urine, wound) and related CPT/HCPCS/CDC terminology; applies to Premera Bluecross outpatient testing decisions.
Multiplex and molecular panel testing may be considered reimbursable in the outpatient setting when criteria in this policy are met, including specific indications for gastrointestinal, respiratory, CSF, and blood pathogen testing.
Testing of urine or wound pathogens is not reimbursable due to insufficient evidence of clinical benefit.
