Coverage Criteria for Presumptive and Definitive Drug Testing

inv-01: Presumptive Screening — Covered Indications

Presumptive urine screening is reimbursable when ANY of the following indications are met:

inv-02: Definitive Testing — Covered Criteria

Confirmatory/definitive qualitative or quantitative drug testing (up to seven drug classes) is reimbursable when ALL of the following are met:

Definitive testing required conditions: 1) Laboratory-based definitive testing is specifically requested and the treating physician documents the rationale in the patient's medical record; AND 2) One or more of the following indications are present: (a) The presumptive screen result conflicts with the patient's history, clinical presentation, or patient statement (e.g., negative for prescribed meds, positive for non-prescribed or illegal drug); (b) Definitive testing is required to diagnose or monitor substance use disorder with frequency limits tied to abstinence duration (0–30 days: up to weekly; 31–90 days: 1–3/month, maximum 3/month; >90 days: 1–3 every three months, maximum 3 per three months); (c) Monitoring individuals on opioid therapy to ensure adherence or detect non-prescribed opioids; (d) A presumptive test does not exist or does not adequately detect the specific drug or metabolite; (e) Definitive identification is needed within a large drug family; (f) A definitive concentration is needed to guide clinical management.<=7 drug classes

inv-03: Not Reimbursable / Exclusions

Not reimbursable when ANY of the following apply:

Not reimbursable: 1) Definitive testing panels larger than seven drug classes; 2) Proprietary tests (e.g., CareView360) or other proprietary LDTs identified in the policy; 3) Any AMA definitive drug class codes (per the policy listing) as stated not reimbursable; 4) Same-day testing of the same drug or metabolite from two different specimens (e.g., blood and urine) by presumptive or definitive analyses; 5) More than one presumptive test result per patient per date of service (including POC plus laboratory presumptive testing on same specimen/date of service); 6) Blanket or routine standing orders for all patients in a practice; 7) Testing requests or situations not specifically addressed by the policy.

inv-04: General coverage stance

Covered when policy criteria are met

Policy-level coverage condition: Presumptive drug screening and definitive drug testing are considered reimbursable only when the policy's specified clinical and documentation criteria are met.

Refer to the detailed coverage criteria, frequency limits, specimen type restrictions, and required documentation in the policy (see earlier chunks).

This policy excludes testing that is mandated by law or regulation and other forensic uses. Specifically, state or federally regulated and legally mandated drug testing (for example, court-ordered or forensic examinations) are out of scope and not addressed by this policy. The policy also does not apply to non-forensic employment-related testing (such as commercial driver licensing or other job‑related pre‑employment or continued‑employment testing) and to testing that is performed solely as a component of urgent or emergency care.

The policy limits covered specimen types to urine or oral fluid for routine presumptive and definitive testing. An exception is made for blood specimens in patients with anuric chronic renal failure, which may be covered. The policy also disallows blanket or routine standing orders for all patients in a practice and notes that more than one presumptive test result per patient per date of service is not reimbursable (e.g., POC and reference lab presumptive testing on the same specimen).

Definitive testing must be ordered with supporting documentation in the medical record; only the specimen types above should be submitted for routine outpatient coverage unless the stated anuric CRF exception applies.

The policy distinguishes validated laboratory-based definitive methods from analyses performed without adequate methodological controls. Coverage descriptions for definitive testing specify use of chromatographic/spectrometric methods (e.g., GC/MS or LC/MS) with stable isotope or other universally recognized internal standards, method- or drug-specific calibration, and matrix-matched quality control material. Tests performed without these elements may be characterized differently for coding (e.g., G0659) and may not meet the policy's definitive-method requirements, risking noncoverage.

The policy recognizes that routine, indiscriminate screening for certain agents provides limited clinical value. Citing clinical studies, it identifies examples where routine pre‑administration screening (for example, routine acetaminophen or salicylate testing in some ED settings) was judged unnecessary and wasteful. As a result, universal or routine screening for low‑yield agents without clinical indication is discouraged and such tests may not be considered reasonable or necessary.

While definitive quantitative urine assays have analytic value, the policy states that quantitative definitive urine testing should not be used to determine drug dosing or to assess adherence to a prescribed dosage regimen because interindividual pharmacokinetic and pharmacogenetic variability limits interpretation. Quantitative testing may be appropriate in select complex cases (for example, to evaluate atypical metabolism or exclude alternative exposure sources), but not as a routine measure of patient adherence.

Pregnancy-related urine drug testing raises special considerations. The policy references ACOG guidance that routine universal UDT in pregnancy is controversial and emphasizes that testing in obstetric contexts should be performed only in compliance with applicable state laws and with patient consent. Clinicians are advised to use validated verbal screening tools and, when laboratory testing is performed, to be aware of their laboratory's test characteristics and request confirmatory testing when appropriate.

Tests that do not meet the policy's stated criteria are not reimbursable. Examples include definitive testing for panels larger than the policy limit, proprietary confirmatory assays identified as excluded, and duplicate or otherwise non‑reasonable testing patterns. The policy states clearly that claims for presumptive or definitive drug testing may be denied if the documented indications, specimen type, ordering, or methodological criteria are not satisfied.

The policy lists specific categories of testing that are considered not covered: definitive testing panels larger than seven drug classes, proprietary or laboratory‑developed proprietary tests (for example, CareView360 as cited), same‑day duplicate testing of the same analyte from different specimens, and multiple presumptive tests for the same patient on the same date of service. Additionally, definitive methods performed without method‑ or drug‑specific calibration, matrix‑matched QC, or stable isotope/internal standards are distinguished (coding G0659) and may not meet definitive‑method coverage standards.

In summary, the policy reaffirms that presumptive and definitive drug tests that fail to meet the coverage criteria and documentation requirements are not reimbursable. Providers should ensure that testing ordered meets the clinical indications, specimen‑type rules, methodological standards, and record documentation described in the policy to avoid denials.

Definitive (laboratory-based, MS- or chromatography-based) testing is preferred for baseline assessment and whenever results will inform major clinical or nonclinical decisions. Immunoassay (presumptive) testing is acceptable in limited circumstances (e.g., low-risk monitoring, payer or institutional requirements, or when rapid point-of-care information is needed), but clinicians should recognize its limitations and obtain confirmatory definitive testing when results are unexpected or will affect treatment, safety, or legal decisions.
