Defines medical necessity, site-of-service review, and drug-specific coverage criteria for intravenous and injectable therapies used to treat a range of autoimmune conditions; applies to providers requesting coverage and site-of-service determination for affected drugs and members (with Alaska fully-insured exception for SOS criteria).

Added criteria for Vyvgart for the treatment of generalized myasthenia gravis (gMG) in adult individuals who are anti-acetylcholine receptor (AChR) antibody positive.
Added criteria for Orencia for the prophylaxis of acute graft versus host disease (aGVHD).
Added criteria for Filspari for the treatment of proteinuria in adults with primary immunoglobulin A nephropathy (IgAN) at risk of rapid disease progression.
