Coverage Criteria for Oral Iron Chelators

Generic deferasirox — transfusional iron overload

Covered when ALL of the following are met for generic deferasirox (transfusional hemosiderosis):

Deferasirox generic - transfusional hemosiderosis: The individual is receiving blood transfusions at regular intervals AND prior to starting deferasirox the individual's serum ferritin level is greater than 1,000 micrograms/liter AND deferasirox is prescribed by or in consultation with a hematologist.serum ferritin > 1,000 mcg/L

Generic deferasirox — non-transfusion dependent thalassemia (NTDT)

Covered when ALL of the following are met for generic deferasirox in non-transfusion dependent thalassemia (NTDT):

Deferasirox generic - NTDT: Individual is aged 10 years or older AND prior to starting deferasirox the individual's serum ferritin level is greater than 300 micrograms/liter AND deferasirox is prescribed by or in consultation with a hematologist.serum ferritin > 300 mcg/L
