HIPEC delivered post‑operatively through an indwelling catheter or port (i.e., administered after the operative session in the inpatient or outpatient setting) is not addressed by this policy.

The role of prophylactic intraperitoneal chemotherapy, HIPEC, or PIPAC for patients with non‑metastatic cancers is considered investigational. Per NCCN guidance, these prophylactic approaches should only be performed in the context of a clinical trial; routine prophylactic use outside trials is not supported by current evidence and is therefore investigational under this policy.

Drugs commonly used during HIPEC (for example, mitomycin, oxaliplatin, carboplatin, and others) have not been approved by the FDA for the HIPEC indication. In addition, several devices have 510(k) clearance for peritoneal lavage (warmed physiologic solution) but none are cleared to administer chemotherapy; the FDA has issued warnings when lavage devices have been marketed for off‑label use in HIPEC.

Use of HIPEC outside the policy’s specified interval cytoreduction criteria is considered investigational. Specifically, HIPEC for ovarian cancer settings beyond the defined interval cytoreductive surgery population (for example, routine use for stage IIIC or IV disease outside the stated interval‑debulking criteria) is investigational and not covered under this policy.

For peritoneal carcinomatosis of colorectal, gastric, and for many cases of recurrent ovarian disease, the evidence base is conflicting or insufficient. Randomized trials and meta‑analyses have produced mixed results — some showing survival benefit with CRS+HIPEC and others showing no benefit or increased adverse events — and overall high‑quality consistent RCT evidence is lacking to uniformly support CRS+HIPEC for these indications.

There is no Medicare national coverage determination (NCD) for HIPEC. Coverage decisions therefore depend on the evidence and the medical‑necessity criteria in this policy and on payer‑specific adjudication.
