Extracorporeal photopheresis (ECP) is addressed differentially across transplant scenarios. The two FDA‑cleared photopheresis systems (UVAR XTS and CELLEX) and the photoactive agent UVADEX (8‑methoxypsoralen) are approved specifically for the palliative treatment of skin manifestations of cutaneous T‑cell lymphoma (CTCL) unresponsive to other therapies. Use of these Therakos/Mallinckrodt systems or UVADEX for conditions other than the FDA‑approved CTCL indications is off‑label and is described in this policy as investigational.

Policy history notes that on 04/08/2013 the policy was replaced and updated with a literature search; at that time a statement was added that ECP is investigational for any other indications not specifically listed as medically necessary. This medical policy does not apply to Medicare Advantage. ECP is not considered medically necessary for prophylactic prevention of graft rejection and for the multiple autoimmune and other indications listed as investigational in this policy. For autoimmune diseases, isolated trials and small studies are insufficiently robust to support medical necessity; therefore these uses remain categorized as investigational or not medically necessary per the evidence review.

Specific transplant‑related uses considered investigational or not established include: ECP for acute graft rejection in lung transplant (evidence limited to small retrospective studies and case series), ECP for graft rejection after liver transplant (small nonrandomized and retrospective studies), and ECP for recurrent graft rejection after kidney transplant (small prospective studies and case reports). Across these organ types prospective randomized trials with larger cohorts are needed to determine net health benefit.

There are no CMS national coverage decisions that support ECP for autoimmune diseases; national Medicare policy does not establish coverage for these indications, which contributes to the investigational stance for autoimmune uses.
