Carbaglu may be considered medically necessary as adjunctive therapy to standard of care for acute hyperammonemia due to propionic acidemia (PA) or methylmalonic acidemia (MMA) when ALL of the following are met:

May be considered medically necessary when ALL of the following are met:

The individual has tried and failed generic sodium phenylbutyrate unless there is a contraindication to generic sodium phenylbutyrate

May be considered medically necessary when ALL of the following are met:

The individual has tried generic sodium phenylbutyrate first and had an inadequate response or intolerance (documentation required)
Applies to branded products Buphenyl, Olpruva, Pheburane as specified
Has a documented dietary protein restriction plan in place
Explicit requirement for branded products and stated for generic as well

Clinical Background

Urea cycle disorders (UCDs) are rare inherited deficiencies of enzymes or transporters required to convert ammonia into urea, including N-acetylglutamate synthase (NAGS), carbamyl phosphate synthetase (CPS), ornithine transcarbamylase (OTC), argininosuccinate synthetase (AS), argininosuccinate lyase (ASL), and arginase (ARG). When these enzymes are absent or deficient, ammonia and upstream metabolites accumulate, producing hyperammonemia that can cause neurologic injury ranging from cognitive impairment to cerebral edema, coma, and death. OTC deficiency is the most common of these disorders, and clinical presentation can occur across the lifespan from newborns to adults.

Management of UCDs is focused on controlling ammonia levels and preventing hyperammonemic crises. Initial acute measures include rehydration, maintaining urine output, stopping protein intake, minimizing catabolism, and removing nitrogen via medications and/or hemodialysis. Chronic strategies center on reducing nitrogen load and enhancing nitrogen disposal through a combination of dietary protein restriction, targeted amino acid supplementation such as arginine or citrulline, and use of nitrogen-scavenging drugs that provide alternative pathways for waste nitrogen excretion.

Specific pharmacologic approaches depend on the enzyme defect. For example, carglumic acid (Carbaglu) addresses NAGS deficiency by restoring the necessary activator for the urea cycle, while agents such as sodium phenylbutyrate or glycerol phenylbutyrate provide alternative nitrogen excretion pathways for individuals with deficiencies of CPS, OTC, or AS.
