ASC Site-of-Service Review and Medical Necessity

Covered when site-of-service review criteria are met

site_of_service_review: Select surgical or diagnostic procedures added to the policy (for example: shoulder arthroscopy, upper gastrointestinal endoscopy, implantable loop recorder implantation, carpal tunnel release techniques) must undergo site-of-service review and are considered medically necessary in an ASC when the specific REVIEW CRITERIA ARE MET in the main policy sections.

The Site of Service medical necessity criteria in this policy do not apply to Indian Health Services (IHS) facilities. This exclusion means that the site-of-service review and associated prior-authorization requirements described elsewhere in this policy are not applied when the procedure is performed at an IHS facility.

Off-campus and on-campus outpatient hospital sites and inpatient hospital sites are considered not medically necessary for the listed elective surgical procedures when the policy's site-of-service criteria are not met. The policy establishes the ambulatory surgical center (ASC) as the preferred medically necessary site for select elective procedures; hospital outpatient or inpatient settings may only be considered medically necessary when an access exception (for example, no qualifying ASC within 30 miles, lack of physician privileges at an ASC, or ASC-specific guideline restrictions) or a clinical-risk exception (for example, ASA classification III or higher, prolonged surgery > 3 hours, significant cardiovascular, pulmonary, liver, renal risks, morbid obesity) is documented and satisfied.
