Upper gastrointestinal (UGI) endoscopy is considered medically necessary when ANY of the following indications are present for individuals aged 19 or older:

Malignant indications: Barrett esophagus (metaplastic columnar or glandular epithelium), dysplasia, esophageal cancer, familial adenomatous polyposis (FAP), gastric cancer, head/neck cancer, history of Lynch syndrome/HNPCC, clinical features highly suspicious for UGI malignancy (e.g., epigastric mass on x-ray, abnormal barium study), prior adenomatous or sessile polyps of the upper GI tract, evaluation for a positive CDH1 mutation, family history of gastric/esophageal/duodenal cancer in a first-degree relative, and tylosis.

Alarm symptoms: Iron deficiency anemia; bleeding (rectal or dark/black stool) suggesting an upper GI source; epigastric mass on examination; persistent vomiting (including hematemesis); dysphagia; unintentional weight loss of >= 3 kg since symptoms started.>= 3 kg weight loss

Follow-up of known non-malignant conditions: Anorexia of unknown cause; evaluation or surveillance related to suspected portal hypertension or cirrhosis (including initial variceal screening and variceal surveillance after eradication); follow-up of eosinophilic or erosive esophagitis (possible Barrett evaluation after 2 months of therapy); therapeutic banding/sclerotherapy for varices with repeat UGI until eradication then surveillance at 1–3 months post-eradication and every 6–12 months thereafter; treatment of bleeding lesions (e.g., ulcers); history of gastric surgery; persistent symptoms despite stopping NSAIDs; persistent non-cardiac chest pain; dysphagia or odynophagia.

Age/risk factors for Barrett detection: Endoscopy may be indicated in men older than 50 years with chronic GERD (>5 years) plus additional risk factors (nocturnal reflux, hiatal hernia, elevated BMI, tobacco use, central adiposity) to detect Barrett esophagus or esophageal adenocarcinoma.

Upper gastrointestinal (UGI) endoscopy is not medically necessary for chronic, non-progressive functional UGI symptoms when the examination is unlikely to change management. This includes situations such as uncomplicated heartburn that responds to conservative medical management, routine screening of the UGI tract in the absence of a clinical indication, evaluation of asymptomatic or resolved radiographic findings (for example, an asymptomatic or therapy-responsive deformed duodenal bulb, uncomplicated duodenal ulcer that has responded to therapy, or an asymptomatic sliding hiatal hernia), and other functional presentations where endoscopic findings would not alter care.

A barium swallow is not recommended to diagnose GERD and should only be used when evaluating structural complications such as strictures or rings (e.g., evaluation of dysphagia or suspected anatomic complication). Endoscopy is generally reserved for alarm features, high-risk screening, refractory symptoms, or evaluation of complications; it is not routinely indicated for symptoms judged to be functional in origin in the absence of warning signs.

Upper GI endoscopy is not medically necessary solely for the purpose of confirming eradication of Helicobacter pylori. The policy explicitly states that UGI performed to confirm H. pylori eradication is considered not medically necessary and should not be used for this purpose.

When the documented clinical presentation does not meet the policy indications for UGI endoscopy, the procedure is considered not medically necessary. Examples include functional disorders without alarm features or failed conservative management, uncomplicated heartburn responsive to conservative care, and any evaluation where endoscopic results will not alter clinical management.

Endoscopy is generally not indicated for functional symptoms in the absence of red flags (alarm symptoms) and is also not indicated for routine surveillance of healed benign disease. Professional society guidance (ASGE) notes that sequential or periodic EGD is generally not indicated for surveillance of healed benign conditions such as esophagitis or healed gastric or duodenal ulcers unless there is a change in clinical status.
