Coverage Criteria for Hospital Beds and Accessories

General hospital bed medical necessity

Positioning beyond ordinary bed: The individual has a medical condition that requires positioning of the body in ways that are not feasible in an ordinary bed (elevation of the head/upper body less than 30 degrees does not usually require the use of a hospital bed).

Frequent repositioning: Requires frequent changes in body positioning.

Head-of-bed elevation >30 degrees: Requires the head of the bed to be elevated more than 30 degrees most of the time because of congestive heart failure, chronic pulmonary disease, or problems with aspiration (pillows or wedges must first be considered and found impractical for reasons other than convenience).

Difficult transfers: Has a medical condition that makes it difficult to transfer from a standard bed to a chair, wheelchair, or to stand (e.g., severe arthritis, lower leg injury, fractured hip, spinal cord injury, disability due to a stroke).

Traction equipment: Requires traction equipment which can be attached only to a hospital bed.

Bariatric/weight-based beds

Heavy-duty / extra-heavy-duty bed criteria:

Heavy duty/bariatric bed: A heavy duty, extra wide/bariatric bed is medically necessary when criteria are met for a fixed-height bed and the individual's weight is more than 350 pounds but less than 600 pounds.

Extra-heavy-duty bed: An extra-heavy-duty bed is medically necessary when criteria are met for a fixed-height hospital bed and the individual's weight is 600 pounds or more.

Air-fluidized bed medical necessity

Air-fluidized bed coverage:

Extensive burns: Air-fluidized bed is medically necessary for treatment of extensive burns (no other criteria required).

Severe pressure sore criteria

Bedridden: The individual is bed-ridden and unable to ambulate fully or partially.
Pressure sore stage: Has stage 3 (full thickness tissue loss) or stage 4 (deep tissue destruction) pressure sore.
Conservative therapy failed: Has exhausted conservative treatment without improvement (such as frequent repositioning, optimization of nutritional status, debridement of wounds, wet to dry dressings) and has used and failed to obtain wound healing from alternative support surfaces (e.g., gel flotation pads, egg crate mattresses, pressure pads and pumps).
Institutionalization avoided: Would require institutionalization in the absence of an air-fluidized bed.
Trained caregiver: Has a trained adult caregiver available to assist with activities of daily living and manage the air-fluidized bed system (including recognition and management of problems such as leakage).
Physician oversight: Has a physician who directs the home treatment regimen and re-evaluates and recertifies the need for the air-fluidized bed monthly.

Devices and accessories not covered

Not covered / not medically necessary devices and accessories:

Beds inappropriate for home use: Continuous lateral rotation beds, institutional type beds, and kinetic therapy type beds are not medically necessary for home use.

Contractual exclusions and non-medical accessories: Adjustable firmness/support mattresses (e.g., Select Comfort), all nonhospital adjustable beds (e.g., Sleep Number, Tempur-Pedic adjustable bases), bed boards/foot boards, bed elevation blocks, overbed tables/trays, positioning pillows/cushions/wedges (except where a related policy applies), power/manual lounge beds, restraints, safety/enclosure/canopy beds, safety accessories, vibrating beds, viscoelastic/memory foam mattresses, and water beds are contractually excluded or not covered.

A number of beds and accessories are contractually excluded from coverage. These noncovered items include adjustable firmness/support mattresses (e.g., Select Comfort), all nonhospital adjustable beds (examples: Craftmatic, Simmons Beautyrest adjustable beds, Sleep Number, Tempur-Pedic adjustable bases), viscoelastic/memory foam mattresses, water beds, vibrating beds, and safety/enclosure/canopy beds (e.g., Cubby Bed, SleepSafe, Posey Enclosure Bed). Also excluded are bed boards/foot boards, bed elevation blocks, overbed tables/trays, positioning pillows/cushions/wedges (except where another related policy applies), power/manual lounge beds, restraints and safety accessories (e.g., belts, harnesses, vests), and other items not primarily intended for medical use.

Certain high-risk institutional devices are explicitly not appropriate for home use and therefore not covered: continuous lateral rotation beds, institutional type beds, and kinetic therapy type beds. The policy lists additional items that may be considered durable medical equipment accessories only when documented as medically necessary (for example, trapeze equipment, bed cradles, and side rails/pads when criteria for a hospital bed are met).

Home use of an air-fluidized bed is considered not medically necessary under any of the following circumstances: the individual requires treatment with wet soaks or has moist wound dressings that are not protected with impervious covering (for example, plastic wrap); the caregiver is unable to provide the required level of care for the air-fluidized system; the structural support of the home is inadequate to support the system (the system may weigh approximately 1600 pounds or more); or the home electrical system or ventilation is insufficient for the anticipated energy use and heat production of the air-fluidized bed system.

Rental, Purchase, and Benefit Limitations

Prior Authorization and Benefit Limitation

Prior authorization and benefit limits apply. Rental or purchase of hospital beds and accessories is subject to the member's benefit as described in the member contract. Rental may be covered for up to 10 months or until the total rental payments equal the purchase price of an equivalent bed, and benefits (rental or purchase) cannot exceed the contracted fee schedule or the total purchase cost as specified in the member contract. Providers should verify member coverage and obtain prior authorization when required by the payer before providing DME.

Prior Authorization

Air-fluidized Bed Conservative Therapy Requirement

Air-fluidized beds require prior trial and failure of conservative therapies before coverage for home use will be approved. Conservative measures include frequent repositioning, optimization of nutritional status, debridement, and appropriate dressings (e.g., dry dressings where clinically indicated). Alternative support surfaces (such as gel flotation pads, egg crate mattresses, pressure pads, and pumps) must have been used without achieving wound healing prior to consideration of an air-fluidized bed for home use.

Background and Rationale

Hospital beds provide adjustable positioning (head, foot, and in some models height) that an ordinary bed does not, and are intended for patients who are largely bedbound or who have medical needs that cannot be met by a standard home bed. They are used in the home setting to permit required positioning, frequent position changes, safe attachment of traction or other medical equipment, and to facilitate transfer or avoid institutionalization when clinically indicated.

Air-fluidized beds are a specialized support surface that create a flotation effect to redistribute body weight and reduce pressure for treatment of extensive burns or certain severe pressure injuries; because of their size and utility they require careful clinical justification and consideration of home safety, structural, electrical, and caregiver support.

Not Covered Items

Adjustable non-hospital home beds and many consumer-grade mattresses and accessories are not covered because they are not primarily intended for medical use. Examples explicitly listed as noncovered include Select Comfort-style adjustable firmness mattresses, Sleep Number and Tempur-Pedic adjustable bases, viscoelastic/memory foam mattresses, water beds, vibrating beds, power or manual lounge beds, bed boards, overbed tables/trays, and positioning pillows/wedges (except where another policy provides coverage, such as infant reflux wedges). These items are contractual exclusions and should be billed accordingly.
