This policy states that two-piece intraoral prosthetic devices or appliances are considered not medically necessary for temporomandibular joint disorder (TMJD). Providers should verify member benefits because some health plan contracts may not include TMJ treatment benefits, and services may be denied if the member contract lacks coverage for TMJ care. (See policy history for the original statement that the two-piece device is not medically necessary.)

The BMJ Rapid Recommendations (BMJ 2023) panel issued guidance on chronic TMJD pain management. The panel favored conservative interventions such as cognitive behavior therapy, therapist-assisted mobilization, manual trigger point therapy, supervised postural or jaw exercise, and usual care with home exercises and education. The panel recommended against or gave conditional recommendations for a number of therapies, including reversible occlusal splints (conditional recommendation), arthrocentesis (conditional), hyaluronic acid/cartilage supplements (conditional), botulinum toxin (conditional), and several pharmacologic and device-based treatments listed in the guideline.

This medical policy applies to Premera Blue Cross membership except where noted; it does not apply to Medicare Advantage. Coverage remains subject to the limits and conditions of the individual member benefit plan, so providers and members should consult the benefit booklet or customer service to confirm applicability and any plan-specific limitations.

The policy lists multiple diagnostic procedures and nonsurgical treatments that are considered investigational and therefore not medically necessary for TMJD. Investigational diagnostic procedures include arthroscopy of the TMJ for purely diagnostic purposes, computerized mandibular scan, electromyography (including surface EMG), joint vibration analysis, kinesiography, neuromuscular junction testing, and thermography. Investigational nonsurgical treatments include biofeedback, botulinum toxin A, dextrose prolotherapy, hyaluronic acid injections, platelet concentrates (e.g., PRP), PENS, TENS, ultrasound, orthodontic services, and various electrostimulation therapies.

Examples of interventions that the BMJ guideline recommended against or gave conditional recommendations for include reversible occlusal splints (conditional recommendation against), arthrocentesis (conditional), hyaluronic acid injections (conditional), botulinum toxin (conditional), and other modalities such as low-level laser therapy, TENS, gabapentin, and certain pharmacologic combinations. Some of these interventions also appear in this policy's investigational or not medically necessary lists.

Conservative therapy first-line

Conservative, reversible therapies are recommended as first-line management for TMJ disorders. Denial risk exists when conservative measures have not been attempted prior to invasive procedures.

Begin with education, home care, supervised jaw exercises, NSAIDs, behavioral therapies, and reversible occlusal appliances or physical therapy as appropriate.
If symptoms persist, consider escalation only after an adequate trial (typically several weeks; surgical consult recommended if no adequate response within ~2–3 weeks for certain conditions per specialty guidance).

Background and context

Temporomandibular joint disorder (TMJD) encompasses pain and dysfunction of the temporomandibular joint and associated masticatory muscles. Causes include disc displacement, degenerative joint disease/arthritis, trauma, or congenital anomalies. Common symptoms are jaw pain, limited or painful range of motion, joint noises (clicking or locking), and functional impairment. Initial management is conservative and reversible — for example, education, soft diet, rest, heat/cold, anti-inflammatory medications, supervised jaw exercises, splints or other reversible oral appliances, and physical therapy — with advanced imaging (CT, MRI) generally reserved for presurgical evaluation or when objective findings warrant further investigation.
