Known or suspected coronary artery disease — clinical indications

Covered when ANY ONE of the following groups of criteria is met for known or suspected CAD:

Angina indications: Stable angina: recurrent angina within 9 months of PCI; CCS class I–II with intolerance of or failure to respond to medical treatment; CCS class III–IV that improves to I–II on medical treatment; CCS class III–IV despite optimal medical treatment. Unstable angina or NSTEMI with high or intermediate risk indicated by elevated troponin, ischemia-related heart failure, persistent hemodynamic or electrical instability, LVEF <40%, prior PCI in past 6 months or prior CABG in past 12 months, suspected or confirmed new ST-segment depression; recurrent angina or ischemia at rest or low activity >20 minutes despite optimal medical therapy; suspected Prinzmetal's (variant) angina; other high-risk clinical contexts as listed in policy.

High risk on noninvasive testing: Echocardiographic wall motion abnormality involving >2 segments; Duke Treadmill Score ≤ -11; resting LVEF ≤35%; stress ECG with ST-segment elevation or ventricular arrhythmia or ≥2 mm ST depression; stress-induced large perfusion defect (particularly anterior) or multiple moderate defects; stress-induced left ventricular dysfunction (exercise LVEF <35%); significant stenosis ≥50% in an unprotected left main coronary artery on recent CCTA.

Noninvasive imaging thresholds that indicate high CAD risk.

Myocardial perfusion imaging high-risk features: A large fixed perfusion defect with left ventricular dilatation or increased lung uptake of radioisotope; a stress-induced moderate perfusion defect with left ventricular dilatation or increased lung uptake; left ventricular enlargement or transient post-stress ischemic left ventricular dilatation.

Prior authorization required for elective procedures and site review — Elective coronary angiography and the site of service will be reviewed for medical necessity. Inpatient hospital site is considered medically necessary only when the individual has clinical conditions that increase risk for complications (examples include significant anesthesia, cardiovascular, pulmonary, renal, liver, bleeding, pregnancy, morbid obesity, or other risks listed in the policy).

Site of service review includes: off‑campus outpatient hospital/medical center, on‑campus outpatient hospital/medical center, ambulatory surgical center, and inpatient hospital/medical center.
Inpatient site is medically necessary only when listed clinical risk factors are present (e.g., ASA class III+, recent MI <3 months, NYHA Class III/IV heart failure, ESRD on dialysis, MELD >8, COPD FEV1 <50%, morbid obesity BMI ≥50, bleeding disorder requiring replacement).

Coronary angiography is indicated/reasonable when noninvasive testing or clinical features indicate high or intermediate risk, or when symptoms are refractory to guideline‑directed medical therapy or other specified clinical indications are present.

Denial Risk

Denial Triggers and Limitations

Denial triggers — Coronary angiography for known or suspected CAD is considered NOT medically necessary in the absence of the specified criteria. Proceeding directly to coronary angiography without appropriate noninvasive risk stratification is considered not medically necessary when based solely on limited factors (e.g., occupation alone, age, or sedentary lifestyle without other policy indications).

Not medically necessary when mild angina is responsive to medication and there is no evidence of ischemia on noninvasive testing.
Occupation alone does not justify bypassing noninvasive testing.
Coronary angiography is not recommended when the patient elects not to undergo revascularization or is not a candidate for revascularization.
