Symptomatic paroxysmal (intermittent) atrial fibrillation that is recurrent (2 or more episodes) as an alternative to medical therapy
Paroxysmal defined as AF returning to normal within 7 days
Symptomatic persistent atrial fibrillation that is refractory or intolerant to one or more antiarrhythmic medications, or there is a contraindication to all appropriate antiarrhythmic drugs
Persistent defined as AF lasting longer than 7 days and not self-terminating

Clinical Background

Catheter ablation—using modalities such as radiofrequency ablation (RFA), cryoablation, or pulsed field ablation (PFA)—is a minimally invasive, catheter‑based procedure that targets the abnormal electrical triggers responsible for atrial fibrillation (AF). The most common focal source of AF is at or near the junction of the pulmonary veins and the left atrium, and procedures commonly aim to achieve pulmonary vein isolation (PVI) to electrically isolate those triggers and restore sinus rhythm. Ablation strategies may include circumferential PVI and, when indicated, additional linear or focal lesions guided by electroanatomic mapping or intracardiac imaging to address non‑PV triggers or substrate that maintains AF. Pulsed field ablation (PFA) is a newer, nonthermal ablation modality that employs brief electrical pulses to ablate tissue. PFA is purported to have myocardial selectivity and to avoid the thermal injury mechanisms associated with RFA and cryoablation, producing a different safety profile; several PFA devices have received U.S. FDA approval for paroxysmal and/or persistent AF. While PFA shares the same procedural aim of isolating pulmonary vein‑left atrial junction triggers, it uses electroporation rather than heat or freezing to create lesions and clinical evidence comparing effectiveness and long‑term outcomes with thermal techniques is evolving.

Repeat procedures after an initial AF ablation are relatively common. Published data and policy reviews estimate that about 30% of individuals will require a follow‑up (repeat) ablation because of recurrent AF or development of atrial flutter. Repeat ablations are often more limited in scope than the initial procedure and should be considered when there is documented recurrence of AF and/or new atrial flutter following the index ablation.
