Defines Premera's coverage and reimbursement stance for laboratory measurement of cardiac biomarkers for diagnosis and prognosis of myocardial infarction and heart failure in acute and outpatient settings; applies to clinicians and labs submitting claims to Premera.
Measurement of B-type natriuretic peptide (BNP) or N-terminal pro BNP is considered reimbursable for diagnosing heart failure in individuals with dyspnea or for assessing severity in chronic heart failure (up to four times per year in the outpatient setting).
All other listed cardiac biomarkers, qualitative troponin testing, ST2 analysis, and any unlisted cardiac biomarkers for myocardial infarction diagnosis or prognosis are not reimbursable due to insufficient evidence of clinical benefit.
