Background & Evidence

Background: Coronary angiography is an invasive fluoroscopic procedure using intravascular contrast to visualize coronary arteries and bypass grafts, quantify ventricular function, and assess coronary blood flow; if an obstructive lesion is identified, percutaneous coronary intervention (PCI) may be performed during the same session or planned later.

Risks: Potential complications include cardiac tamponade, arrhythmia, injury to a catheterized artery, low blood pressure, allergic reaction to contrast dye, excessive bleeding, kidney damage, stroke, or myocardial infarction.

Epidemiology and guideline support: Coronary artery disease is the most common form of heart disease in the U.S. (hundreds of thousands of deaths annually and ~805,000 myocardial infarctions per year). Specialty society guidelines (ACC/AHA/ESC and related documents) support coronary angiography in specific high-risk scenarios such as acute coronary syndromes, high-risk stress test results, suspected left main or severe multivessel disease on noninvasive imaging, and for risk assessment when noninvasive testing is inconclusive or contraindicated.

When coronary angiography is considered NOT medically necessary:

Coronary angiography for known or suspected CAD is NOT medically necessary in the absence of the above criteria

Coronary angiography discouraged for mild angina responsive to medication with no ischemia on noninvasive testing

Occupation alone (even with sedentary lifestyle) without other listed diagnoses or abnormal noninvasive testing is not sufficient to proceed to coronary angiography

Medical records submitted for review should document that medical necessity criteria are met and include:

Elective coronary angiography and the site of service will be reviewed for medical necessity. Preferred outpatient sites include off-campus outpatient hospital/medical center, on-campus outpatient hospital/medical center, and ambulatory surgical center. Inpatient hospital/medical center is considered medically necessary only when the individual has one or more listed increased-risk conditions (examples include anesthesia risks, significant cardiovascular risks such as recent MI <3 months or uncompensated CHF NYHA III/IV, advanced liver disease, severe pulmonary disease, ESRD on dialysis, morbid obesity, pregnancy, bleeding disorders, or anticipated need for transfusion).
