This policy defines medical necessity criteria for transcatheter catheter ablation procedures for atrial fibrillation, including radiofrequency ablation (RFA), cryoablation, and pulsed field ablation (PFA). Medical Necessity Criteria

Initial Therapy - Medical Necessity

Transcatheter RFA, cryoablation, or pulsed field ablation may be considered medically necessary as a treatment for either of the following:

ANY of the following

Symptomatic paroxysmal (intermittent) atrial fibrillation that is recurrent (2 or more episodes) as an alternative to medical therapy.
Symptomatic persistent atrial fibrillation that is refractory or intolerant to one or more antiarrhythmic medications (or there is a contraindication to all appropriate antiarrhythmic drugs).

Repeat Procedure - Medical Necessity

Repeat RFA, cryoablation, or pulsed field ablation may be considered medically necessary when:

ALL of the following

There is recurrence of atrial fibrillation and/or development of atrial flutter following the initial procedure.
Operational note: Clinical factors influencing need for repeat include length of AF, permanent AF, left atrial size, and left ventricular ejection fraction; repeat procedures are generally more limited in scope than the initial procedure (contextual operational note).

Not Medically Necessary

Transcatheter RFA, cryoablation, or pulsed field ablation to treat atrial fibrillation is considered not medically necessary when:

Cases of atrial fibrillation that do not meet the criteria outlined for medical necessity above (i.e., not symptomatic paroxysmal recurrent >=2 episodes or not symptomatic persistent refractory/intolerant/contraindicated to antiarrhythmic drugs).

Contact force–sensing irrigated catheters (e.g., TactiCath) supported by randomized trials cited.

Must document symptomatic AF type (paroxysmal vs persistent), number of recurrent episodes (>=2 for paroxysmal alternative), prior antiarrhythmic medication trials and intolerance/contraindications if claiming refractory persistent AF, and recurrence/atrial flutter evidence for repeat procedures.

Symptomatic paroxysmal AF: recurrent defined as 2 or more episodes (>=2).

Background and Evidence Summary

Catheter ablation for atrial fibrillation is a minimally invasive approach that targets focal triggers (most commonly at the pulmonary vein–left atrial junction) or modifies the atrial substrate to restore sinus rhythm. Pulmonary vein isolation (PVI) is described as the cornerstone lesion set for most AF ablation procedures, although additional lesion sets (linear lesions, focal triggers outside the veins, complex fractionated electrogram ablation, ganglionated plexi ablation) may be performed based on mapping and patient substrate. Repeat procedures are common — as many as ~30% of individuals may require follow-up ablation for recurrent AF or development of atrial flutter — and these repeat procedures are typically more limited in scope than the initial procedure. Part 1 of this policy contains the clinical coverage criteria and coding guidance; Part 2 provides the trial evidence, systematic reviews, and regulatory status supporting these statements.
