Submission and Physician Attestation Requirements

Submission requirements: Complete all required sections of the attached Dependent Disability Certification form. The subscriber must complete and sign applicable fields. A certified physician must complete, sign, and certify Section 3 (Physician's Information). The physician MUST include the ICD-10 diagnosis code(s) and the Date of Patient's Last Exam (must be within the past year). Mail the completed form and all supporting documents to the address on the form. The subscriber is responsible for providing accurate signatures and required attachments; providing false or misleading information is a crime and may result in penalties, fines, imprisonment, and denial of benefits.

Physician must complete and sign Section 3 and include ICD-10 diagnosis and Date of Last Exam (within past 12 months).
Subscriber must sign where indicated before proceeding when SSDI/guardianship documents are not provided.
Mail completed form and attachments to the address shown on the form; retain copies for your records.

Supporting Documents Flow

Supporting documents flow: Determine which path applies and attach the required documents. If the dependent has been declared disabled by the Social Security Administration (SSDI or SSI), attach a copy of the SSDI award letter and, if applicable, the most recent monthly SSI statement. If the dependent has been placed in legal guardianship, attach a copy of the active court order. If SSDI/SSI award letter or court order is not available, the treating physician must complete and sign the physician attestation (Section 3). In all cases where SSDI or guardianship documents are provided, the subscriber must sign the subscriber signature line and STOP (no physician attestation required unless specified). If the answer to SSA disability or guardianship questions is No, the subscriber must sign and then continue to Section 3 for physician completion.

If Yes — SSA disability declared: attach SSDI award letter and SSI statement (if applicable); subscriber signs and STOP.
If Yes — Legal guardianship: attach active court order; subscriber signs and STOP.
If No to either SSDI or guardianship: subscriber signs and continue to Section 3; physician completes Section 3.
If SSDI/guardianship documents are not available, physician must complete and sign the medical attestation (Section 3).
Subscriber signature is required on the subscriber signature line in all paths indicated on the form.
