Prior authorization coverage stance
Prior authorization is required for the listed behavioral health services and codes except for emergency services. All out‑of‑network (non‑participating) providers must obtain prior authorization prior to providing behavioral health services.
Services requiring prior authorization
- Acute mental health inpatient services (Revenue codes: 0114, 0124, 0134, 0144, 0154, 0204).
- Observation / Crisis Stabilization / Holding Bed (Revenue codes: 0761, 0154).
