Benefits may vary between groups and contracts; providers and reviewers must refer to the member's Benefit Booklet, Evidence of Coverage, or Subscriber Agreement to determine applicable benefit limitations, exclusions, and surgery benefits.

Services that are determined to be not medically necessary are not covered. Providers may not bill the member for such services unless the member has been informed and has given prior written consent to assume financial responsibility for the treatment.

Definition of BCBSRI Contracted Primary Care Provider

DefinitionA BCBSRI Contracted Primary Care Provider is a credentialed clinician in one of the specified specialties (Internal Medicine; Pediatric Medicine; Family Practice; Obstetrics and Gynecology; Doctor of Osteopathic Medicine; NP/PCP; PA) who is eligible for an exemption from prior authorization for Fully-Funded Commercial Products when ordering spinal procedures.

Applicable productsFully-Funded Commercial Products only (effective 10/01/2025) — exemption does not apply to Self-Funded Commercial or Medicare Advantage Plans, which still require prior authorization.
