Coverage Criteria for Unlisted/Unspecified Codes

ALL of the following

Neighborhood may use LCDs, NCDs, Interqual, Rhode Island EOHHS recommendations, Clinical Medical Policies, and medical record documentation to determine medical necessity.

Providers must bill an unlisted/unspecified code only when no other code is appropriate (use the closest or most similar code).

Claims for services billed with unlisted/unspecified or 'not otherwise classified' codes must include supporting documentation sufficient for consideration of payment.

Documentation in the medical record must support the service(s) billed and the medical necessity; the report should include: nature and extent of the procedure; total time, effort, and equipment used; medical necessity; complexity of symptoms, final diagnosis, physical findings, concurrent problems, and follow‑up care.

Neighborhood reserves the right to request medical records for any service billed and may audit services submitted for payment; payment is subject to member eligibility, benefit coverage, contractual terms, and applicable regulations.

Procedures or devices deemed experimental or not FDA approved are not covered.

Claims must meet AMA CPT, ICD‑10‑CM, and HCPCS Level II coding standards and be submitted within contractually determined timely filing guidelines.
