Obstetrical coverage criteria

Neighborhood reimburses global maternity services with specified inclusions and limits; certain services are separately reimbursed or excluded.

Global OB package includes the following services when billed with global maternity CPT codes (e.g., 59400, 59510, 59610, 59618): initial and subsequent history; recording of weight; physical examination; blood pressure checks; monitoring of fetal heart tones; routine chemical urinalysis; routine prenatal visits; postpartum hospital and office visits following vaginal and cesarean delivery; management of uncomplicated labor and delivery services, including admission.

Confirmation of pregnancy visits: a minimal confirmation visit (for example, blood test and prescription of prenatal vitamins) is included in the global package when the OB record is started the same day; if pregnancy was already confirmed by another provider the confirmation visit is not separately billable.

Antepartum/postpartum billing options

Antepartum care only — 1-3 visits: bill using appropriate E/M codes.
Antepartum care only — 4-6 visits: bill CPT 59425.
Antepartum care only — 7 or more visits: bill CPT 59426.
Postpartum care only: bill CPT 59430.

Multiple births and delivery sequencing: For multiple vaginal deliveries, bill Baby A with the global vaginal delivery code and bill subsequent infants (B+) using the vaginal delivery only code as applicable (multiple procedure payment rules apply). For deliveries involving both vaginal and cesarean components, bill according to the primary procedure (cesarean billed as primary when appropriate) and follow the billing guidance for cesarean delivery codes and modifiers.

Services unrelated to pregnancy and medical complications: services unrelated to pregnancy provided by the global maternity provider (for example, evaluation and management for non-pregnancy conditions) and medical complications (including but not limited to hypertension, toxemia, hyperemesis gravidarum, congenital/genetic defects, cardiac or neurological abnormalities) must be documented and coded separately and are reimbursed outside the global obstetrical package.

Obstetric ultrasound limits and indications: Neighborhood will reimburse up to three routine standard ultrasounds during each pregnancy. Additional limited or follow-up ultrasounds will be reimbursed only when a diagnosis or condition is suspected that represents an abnormality of pregnancy or represents a threat to the fetus or the delivery.Up to 3 routine standard ultrasounds per pregnancy

Exclusions: The following are not covered — 3D/4D ultrasounds (considered investigational), pre-planned home births, water births, services provided by a doula, ultrasounds provided solely for fetal pictures, and routine gender-determination ultrasounds absent a documented medical concern.

Documentation and claims: providers must submit complete claims within timely filing guidelines; Neighborhood may request medical records. Coding must follow CPT, ICD-10-CM, and HCPCS, and documentation should support any services billed separately from the global package.

Early Maternity Discharge service codes (for example, CPT 99501, 99502 and related nursing/home care HCPCS codes T1001–T1030) must be billed with one of the diagnosis codes listed in Table 2 to support payment for those services.

Providers should follow CPT coding guidance for multiple gestations and list additional-gestation ultrasound codes in addition to the primary procedure as required by CPT (e.g., use of 76802, 76810, 76812, 76814 when applicable).

Neighborhood may use LCDs, NCDs, industry-accepted criteria (e.g., InterQual), EOHHS recommendations, and Neighborhood Clinical Medical Policies (CMP) to determine medical necessity. It is the provider's responsibility to verify member eligibility, coverage, and any required prior authorization or medical necessity criteria BEFORE rendering services.
