Coverage criteria for unlisted/unspecified codes

Covered when ALL of the following conditions are met:

Medical necessity: All services must be medically necessary; Neighborhood may use National Coverage Determination (NCD), Local Coverage Determination (LCD), industry-accepted criteria such as Interqual, Rhode Island EOHHS recommendations, Clinical Medical Policies (CMP), and medical record documentation to determine necessity. It is the provider's responsibility to verify eligibility, coverage and authorization prior to rendering services.
Appropriate coding: Unlisted/unspecified procedures should only be billed when no other code is appropriate; providers should bill with the closest or most similar unlisted/unspecified code and coding must follow CPT, ICD-10-CM, and HCPCS standards.
Supporting documentation: Services billed with unlisted/unspecified or 'not otherwise classified' codes require supporting documentation and Neighborhood may request medical records; documentation must support the services billed and the medical necessity and should describe nature/extent of procedure, medical necessity, total time/effort/equipment, and complexity including symptoms, diagnosis, findings, concurrent problems, and follow-up care.
Exclusions: Procedures or devices deemed experimental or not FDA approved are not covered.

Provider verification (operational): Providers must verify member eligibility, coverage, and authorization requirements prior to rendering services; refer to plan-specific prior authorization reference and Neighborhood Clinical Medical Policies as needed.
