Obstetrical care is considered included in the obstetrical package from the date the obstetrical medical record is established.

Routine obstetrical care (global package) includes: initial and subsequent history; physical examination; recording of weight; blood pressure checks; routine chemical urinalysis; monitoring of fetal heart tones; routine visits; management of uncomplicated labor and delivery including admission; and postpartum hospital and office visits following vaginal and cesarean delivery.

High-risk visits or visits for an unrelated diagnosis must be billed with supporting diagnosis codes for separate reimbursement to be considered.

When multiple vendors provide routine care, Neighborhood will reimburse up to the total allowed global package amount; appropriate modifiers should be used to indicate a separately identifiable service and notes may be required to support separate reimbursement.

Nutritional counseling within global period

Nutritional counseling may be reimbursed within the global period when rendered on the same Evaluation and Management service by the same provider.
A pregnancy-related diagnosis must be billed on the claim and appropriate modifiers and notes may be required to support separate reimbursement.

Multiple deliveries coding

Both vaginal deliveries (twin births): code Twin A (first born) as 59400 and Twin B as 59409 with modifier -51.
Both cesarean deliveries: only one cesarean surgical procedure is billed; code 59510 once with modifier -22.
Mixed delivery (one vaginal, one cesarean): code 59510 for the cesarean (primary procedure) and 59409-51 for the vaginal delivery.
If more than two newborns are delivered, each additional baby is coded by type of delivery with the appropriate modifier.
Medical complications (e.g., hypertension, toxemia, hyperemesis gravidarum, congenital/genetic defects, cardiac or neurologic abnormalities) should be documented and coded appropriately.

Neighborhood may reimburse for nutritional counseling provided on the same E/M service by the same provider during the global period when pregnancy-related diagnosis codes are billed. Appropriate modifiers should be used to indicate a separately identifiable service; documentation/notes may be required to support separate reimbursement.

Multiple deliveries coding guidance

For multiple deliveries follow coding by delivery type based on mode of delivery and birth order. Document medical complications (e.g., hypertension, toxemia, hyperemesis gravidarum, congenital/genetic defects, cardiac or neurological abnormalities) in the record and code appropriately. This guidance applies to both CMS-1500 and UB-92 claim submissions.

Both vaginal deliveries: code Twin A (first born) as 59400 and Twin B as 59409 with modifier -51.
Both cesarean deliveries: bill 59510 once for the cesarean procedure; report increased procedural services when applicable (e.g., modifier -22) as appropriate.
One vaginal and one cesarean: bill 59510 for the cesarean (primary procedure) and 59409 with modifier -51 for the vaginal delivery.
More than two newborns: code each additional newborn by the type of delivery with the appropriate modifiers.
Ensure mode of delivery information and birth order are clearly documented to support coding choices.
