Coverage includes the following implant types when they meet general guidelines and plan review/contracting requirements:

Coverage excludes specific implant types. Dental implants are not covered because dental services are excluded. Implants for cosmetic reasons are excluded. Breast implants are excluded except when used for reconstruction after mastectomy. Implants for gender reassignment are not covered. Prior Authorization Required

Prior Authorization Required. Certain implantable devices require preauthorization before services are performed. Providers must obtain prior authorization to ensure the device is covered, reimbursement is appropriately contracted, and claims can be adjudicated. Contact Neighborhood Health Plan of Rhode Island Provider Services or use the payer’s preauthorization portal to submit requests before scheduling implantation.

Dental implants are not covered (dental services are excluded).

Clinical rationale and documentation: history, diagnosis, prior conservative therapies, objective findings, and the anticipated benefit of the implant.
Operative reports, device model/manufacturer details, and FDA approval status (if applicable).
Evidence of payer or NHPRI contracting/price agreement when required for the specific device.
Copies of prior authorization determinations and any peer-review correspondence.
For Extended Family Planning members: documentation must show the implant is an approved contraceptive product covered under the limited benefit.

Verify member’s benefit package (Rite Care, CSHCN, Substitute Care, Rhody Health Partners/Options Phase One, ACA Adult Expansion) prior to ordering the implant.
Do not bill or schedule non-covered implants; obtain alternative care plans when device is excluded.
