Main requirements: A. The requested drug/product is being used for an FDA-approved indication or a medically accepted indication as supported by AHFS-DI, Thomson Micromedex DrugDex, Clinical Pharmacology, Wolters Kluwer Lexi-Drugs, or peer-reviewed published medical literature; B. The prescribed dose and quantity fall within FDA-approved labeling or compendium-supported dosing guidelines; C. At least one of the justifications (a–f) below is met.

Items A and B are mandatory; item C requires one of the sub-conditions a–f.

Justifications for exceeding quantity limits: a. Loading dose for initiation of therapy; b. Weight- or BSA-based dosing requiring greater quantity (provide patient's weight and/or BSA); c. Need for additional quantities of a lower strength due to intolerance to recommended maintenance dosing with higher strength (provide rationale); d. Dose or frequency adjustment due to a drug–drug interaction (provide specific interaction); e. Continuation of therapy on the requested dose where patient is stable and not experiencing adverse side effects (provide start date of therapy on requested dose); f. Failure of lower doses necessitating a higher dose (provide failed regimens).
