Quantity Limit Exception — Coverage Criteria
This policy defines the clinical conditions under which an exception to a drug quantity limit may be authorized. Authorization is considered only when the request meets all elements of Section I: the drug is for an FDA‑approved or medically accepted indication, the requested dose and quantity align with FDA labeling or compendia-supported dosing, and at least one specific justification in Section I.C is documented.
